摘要
目的了解肺结核(TB)合并糖尿病(TB-DM)患者发生乙胺丁醇耐药的相关基因分子特征。方法通过TB-DNA、TR-RNA和抗酸染色(结果均为阳性)筛选出2017年12月至2019年7月该院就诊的763例结核病患者,将其分为TB组和TB-DM组,经PCR-反向点杂交法检测耐药相关基因embB,对比分析2组基因突变特点,并搜集相关临床资料以探讨其与乙胺丁醇耐药发生的相关性。结果TB组的525例患者中19例发生乙胺丁醇耐药,TB-DM组的207例患者中13例发生乙胺丁醇耐药。2组发生乙胺丁醇耐药突变时,embB306N突变率(>84.62%)最高。结论TB组乙胺丁醇耐药患者易发生embB306N和embB306M2位点突变,TB-DM组易发生embB306M1位点突变。
Objective To understand the molecular characteristics of genes related to ethambutol-resistance in patients with tuberculosis(TB)and diabetes(TB-DM).Methods A total of 763 patients with TB admitted to the hospital from December 2017 to July 2019 were screened by TB-DNA,TR-RNA,and acid-fast staining(all were positive).The patients were divided into TB group and TB-DM group.The ethambutol related gene embB characteristics of gene mutations in the two groups were compared and analyzed by PCR-reverse dot hybridization.The clinical data were collected to explore its correlation with the occurrence of ethambutol-resistance.Results Nineteen of the 525 patients in the TB group developed ethambutol-resistance,and 13 of the 207 patients in the TB-DM group developed ethambutol-resistance.The embB306N mutation rate(>84.62%)was the highest.Conclusion The embB306N and embB306M2 mutations in TB patients with ethambutol-resistance are prone to occur,the embB306M1 mutation is prone to occur in the TB-DM patients with ethambutol-resistance.
