摘要
目的:观察双合汤加减方联合温针灸治疗膝骨性关节炎(knee osteo arthritis, KOA)的疗效。方法:选取2018年3月~2019年3月入我院就诊的KOA患者80例,通过随机数字法分为观察组和对照组,各40例。对照组患者接受常规基础治疗,观察组在对照组基础上使用温针灸疗法辅以双合汤加减方治疗,两组均干预两个月。比较两组患者干预后疗效;干预前后的膝关节协会评分(knee society knee score, KSS)、疼痛等级评定量表(Visual Analog Scale, VAS)评分、生活质量量表(short-form 36 health survey scale, SF-36)总分、血清肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)和白介素-6(Interleukin-6,IL-6)水平变化情况;术后并发症发生情况。结果:治疗2月后,观察组总有效率高于对照组(P<0.05);KSS评分明显高于对照组(P<0.05),VAS评分明显低于对照组(P<0.05);干预后(即时)、干预后6月SF-36总分明显高于对照组(P<0.05);干预后TNF-α和IL-6水平显著低于对照组(P<0.05);干预期间各种不良反应总发生率与对照组无明显差异(P>0.05)。结论:双合汤加减方联合温针灸用于KOA患者对膝关节功能、疼痛改善,提高KOA疗效和生活质量有积极意义。
Objective: To study the efficacy of using Shuanghe Decoction combined with warm acupuncture in the treatment of knee osteoarthritis(KOA).Methods: 80 patients with KOA admitted to our hospital from March 2018 to March 2019 were randomly divided into observation group(n=40) and controlled group(n=40).Patients in the controlled group received routine basic treatment, while those in the observation group received warm acupuncture plus Shuanghe Decoction on the basis of the controlled group.Both groups were intervened for two months.The curative effect was compared between the two groups after intervention;knee society association score(KSS),pain rating scale(VAS),total score of quality of life scale(short-form 36 health survey scale, SF-36),serum tumor necrosis factor-α(TNF-α) and interleukin-6(IL-6) levels were compared before and after intervention, and postoperative complications were observed.Results: After 2 months of treatment, the total effective rate of the observation group was significantly higher than that of the controlled group(P<0.05).KSS score of the observation group was significantly higher than that of the controlled group(P<0.05).The total score of SF-36 after intervention(immediately) and 6 months after intervention was significantly higher than that of the controlled group(P<0.05),and the levels of TNF-α and IL-6 after intervention were significantly lower than those of the controlled group.There was no significant difference in the total incidence of adverse reactions between the intervention group and the controlled group(P>0.05).Conclusion: Shuanghe Decoction combined with warm acupuncture has positive significance in improving knee joint function, pain, curative effect of KOA and quality of life in patients with KOA.
