摘要
目的:观察柴附解郁方联合艾司西酞普兰治疗抑郁症的临床疗效。方法:选择本院2018年1月~2020年1月收治的抑郁症患者110例,按照随机数字表法分为2组,每组55例,对照组予以艾司西酞普兰治疗,治疗组予以柴附解郁方联合艾司西酞普兰治疗,疗程均为6周。比较两组治疗前后汉密尔顿抑郁量表(HAMD)评分、抑郁自评量表(SDS)评分和负性认知加工偏向问卷(NCPBQ)评分,并比较两组治疗前后血清5-羟色胺(5-HT)、多巴胺(DA)、皮质醇(Cor)、孤啡肽(OFQ)、星型胶质源性蛋白(S100B)、髓鞘碱性蛋白(MBP)、脑源性神经生长因子(BDNF)、胰岛素生长因子-1(IGF-1)水平,比较两组临床疗效。结果:治疗后治疗组HAMD评分、SDS评分及NCPBQ各维度评分均低于对照组(P<0.01),血清5-HT、DA、BDNF水平高于对照组(P<0.01),Cor、OFQ、S100B水平低于对照组(P<0.01),临床疗效优于对照组(P<0.01),差异均有统计学意义。结论:附解郁方联合艾司西酞普兰可改善神经递质和神经功能相关因子含量,有效缓解抑郁症状,改善负性认知状况,治疗抑郁症效果显著。
Objective:To investigate the clinical efficacy of using Chaifu Jieyu Decoction combined with Escitalopram in the treatment of depression.Methods:110 patients with depression admitted to our hospital from January 2018 to January 2020 were selected and divided into 2 groups according to the random number table method,with 55 patients in each group.The controlled group was treated with Escitalopram,and the treatment group was treated with Chaifu Jieyu Decoction combined with Escitalopram.Both groups were treated for 6 weeks,Hamilton depression scale(HAMD)scores,depression self-rating scale(SDS)scores and negative cognitive processing bias questionnaire(NCPBQ)scores before and after treatment were compared between the two groups,and serum levels of 5-hydroxytryptamine(5-HT),dopamine(DA),cortisol(Cor),orphanin FQ(OFQ),astro-derived protein(S100 B),myelin basic protein(MBP),brain-derived neurotrophic factor(BDNF)and insulin growth factor-1(IGF-1)were compared between the two groups before and after treatment,compared two groups of clinical curative effect.Results:After treatment,HAMD scores,SDS scores and NCPBQ scores in the treatment group were lower than the controlled group(P<0.01),serum levels of 5-HT,DA and BDNF were higher than the controlled group(P<0.01),levels of Cor,OFQ and S100 B were lower than the controlled group(P<0.01),and the clinical efficacy was better than the controlled group(P<0.01).The differences were statistically significant.Conclusion:Chaifu Jieyu Decoction combined with Escitalopram can improve the content of neurotransmitters and neurofunctional factors,effectively relieve the symptoms of depression,improve the negative cognitive status,and have a significant effect on the treatment of depression.
