摘要
目的:分析薄层液基细胞学检查(TCT)及HPV检测在子宫颈癌筛查中的应用价值。方法:回顾分析2018年1月-2020年2月本院800例进行子宫颈癌筛查的疑似子宫颈癌前病变患者临床资料,均给予TCT与HPV检测,以病理学检验结果为准,比较两种检验方法的检测结果,分析TCT、HPV检测和病理学检验结果的一致性。结果:本组800例受检者中,病理学检验确诊子宫颈癌患者66例。TCT检测的灵敏度为30.30%(20/66),特异度为99.46%(730/734),准确率为93.75%(750/800),HPV检测的灵敏度为69.70%(46/66),特异度为97.00%(712/734),准确率为94.75%(758/800);HPV检测子宫颈癌的灵敏度较TCT高,特异度较TCT低,差异有统计学意义(χ^(2)=20.485、12.686,P<0.05),但是两种方法的诊断准确率比较差异无统计学意义(χ^(2)=0.738,P>0.05);TCT、HPV诊断结果与病理学诊断结果之间有良好的一致性(Kappa>0.7)。结论:TCT与HPV检测都可有效筛查出子宫颈癌患者,准确率较高,但是在实际检验过程中,必要可进行联合性诊断,以此更好地为临床医师精确诊断患者病情提供参考依据。
Objective:To analyze the application value of thinprep cytologic test (TCT) and HPV detection in cervical cancer screening.Method:The clinical data of 800 patients with suspected precancerous cervical lesions who underwent cervical cancer screening in our hospital from January 2018 to February 2020 were retrospectively analyzed,and all of them were given TCT and HPV detection.Based on the pathological examination results,the results of the two test methods were compared to analyze the consistency between the TCT,HPV test and the pathological examination results.Result:Sixty-six cases of cervical cancer were confirmed by pathological examination in 800 subjects.The sensitivity of TCT detection was 30.30% (20/66),the specificity was 99.46% (730/734),the accuracy rate was 93.75% (750/800),and the sensitivity of HPV detection was 69.70% (46/66),the specificity was 97.00% (712/734),the accuracy rate was 94.75% (758/800).The sensitivity and specificity of HPV in the detection of cervical cancer were higher than those of TCT,and the differences were statistically significant (χ^(2)=20.485,12.686,P<0.05),but there was no statistically significant difference in the diagnostic accuracy rate between the two methods (χ^(2)=0.738,P>0.05).There were good agreement between TCT,HPV diagnosis and pathological diagnosis (Kappa>0.7).Conclusion:TCT and HPV detection can effectively screen cervical cancer patients with high accuracy,but in the actual testing process,it is necessary to carry out combined diagnosis,so as to better provide reference for clinicians to accurately diagnose patients’ conditions.
