摘要
目的:分析和探讨老年胸腰椎压缩性骨折患者应用MRI与CT检查效果。方法:选择2018年3月-2020年4月来笔者所在医院接受诊疗的老年胸腰椎压缩性骨折患者82例开展此次研究,且所有选取患者均接受MRI(磁共振)检查、CT检查,对比其最终的检查诊断结果。结果:(1)将病理检查结果作为金标准,在MRI检查、CT检查后,82例患者的诊断符合率分别为91.46%、86.59%;(2)MRI检查后,82例老年胸腰椎压缩性骨折患者中共检出骨折椎体95个,其中38个为陈旧胸腰椎压缩性骨折,占比40%,MRI表现为:T1、T2加权图像均呈等信号,部分可见低信号硬化边,抑脂T2加权图像呈等信号,周围软组织无异常信号出现;另外57个则为新鲜胸腰椎压缩性骨折,占比60%,MRI表现为:T1加权图像呈低信号,T2加权图像与压脂T2加权图像均呈高信号,周围有骨髓水肿发生。结论:与CT检查相比,MRI检查在老年胸腰椎压缩性骨折患者中的应用,可辨别其椎体形态及骨折的新旧程度,使用价值较为显著,值得在临床中推广。
Objective:To analyze and discuss diagnosis effect of MRI and CT in elderly patients with thoracolumbar compression fracture.Method:From March 2018 to April 2020,82 elderly patients with thoracolumbar compression fractures who came to our hospital for diagnosis and treatment were selected to carry out this study,and all the selected patients were examined by MRI and CT,and the final diagnosis results were compared.Result:(1) Taking pathological examination results as the gold standard,after MRI examination and CT examination,the diagnostic coincidence rates of 82 patients were 91.46% and 86.59%,respectively.(2) After MRI examination,95 fractured vertebral bodies were detected in 82 elderly patients with thoracolumbar compression fractures,of which 38 were old thoracolumbar compression fractures,accounting for 40%.MRI showed that:T1 and T2 weighted images showed isosignals,some showed low-signal hardening edges,fat suppression T2 weighted images showed isosignals,and no abnormal signals appeared in surrounding soft tissues.The other 57 cases were fresh thoracolumbar compression fractures,accounting for 60%.MRI showed that T1-weighted images showed low signal,T2-weighted images and fat-pressing T2-weighted images showed high signal,and bone marrow edema occurred around them.Conclusion:Compared with CT examination,MRI examination in the elderly patients with thoracolumbar compression fracture can distinguish the shape of the vertebral body and the old and new degree of fracture,which has a significant value and is worthy of clinical promotion.
