摘要
目的利用数据挖掘技术,分析刘雁峰教授治疗卵巢储备功能下降(DOR)的用药规律,归纳刘教授辨治本病的经验。方法收集2019年1月1日至12月31日就诊于刘雁峰教授门诊的DOR患者的处方资料,建立数据库,通过频次统计、复杂网络分析总结用药特点。结果本研究共纳入84名患者,251张处方。使用频次较高的药物有女贞子、枸杞子、黄精、升麻、熟地黄、葛根、郁金等;药性频次最高的为寒凉类药物,温热类、平性药物亦占较大比重;药味频次最高者为甘味,其次为辛味、苦味;归经频次最高者为肝经,其次为肾经等。复杂网络分析得出的核心药物共有20味,其构成的核心处方体现了补虚、解郁、活血、升清、清热等治法。结论刘雁峰教授立足DOR肾精亏虚、肝郁血虚之基本病机,治以益肾填精、养血疏肝,兼调气血运行,强调寒温并进、补通兼顾、升降并举,从而燮理阴阳平衡、动静平衡、升降平衡,促进患者卵巢储备功能的恢复和改善。
Objective To analysis the prescription regularity of Professor Liu Yanfeng in treating diminished ovarian reserve(DOR)with data mining technology and to summary Prof.Liu′s experience in treating DOR.Methods The medical records of DOR patients treated by Prof.Liu in Gynecology Clinic of Dongzhimen Hospital from January 1 st to December 31 st 2019 were collected and screened.Data of eligible records were extracted and the database was established for data mining,including frequency statistics and complex network analysis,to summarize prescription characteristics.Results A total of 251 prescriptions of 84 DOR patients were included.The frequently-used Chinese medicine included Nuzhenzi(Fructus LigustriLucidi),Gouqizi(Fructus Lycii),Huangjing(Rhizoma Polygonati Sibirici),Shengma(Rhizoma Cimicifugae Foetidae),Shudihuang(Radix Rehmanniae Praeparata),Gegen(Radix Puerariae Lobatae),Yujin(Radix Curcumae Wenyujin),et al.As for property statistics,cold and cool drugs were most frequently used,while warm and hot drugs and mild drugs also had large proportions.Sweet drugs had highest frequency in flavor statistics.Inmeridian distribution statistics,liver and kidney meridians had higher frequency than other meridians.There were 20 core drugs got from complex network analysis which embodied the efficacy of tonification,soothing the liver,activing blood,upraising qi,clearing heat.Conclusions Prof.Liu′s treatment for DOR is based on the methods of tonifying the kidney,regulating the liver and promoting the circulation of qi and blood,according to the basic pathology of kidney deficiency,liver qi depression,blood deficiency.She emphasizes on the simultaneous use of cold and warm drugs,tonifying and purging drugs,and upwards and downwards drugs.The therapy harmonizes the balance of yin and yang,motion and quiescence,and ascending and descending movements to promote ovarian reserve.
