摘要
目的利用结构方程模型构建武汉火神山医院一线医务人员战疫时期身心健康状态模型。方法选取2020年2月17—29日期间以部队医院支援武汉火神山医院救治新型冠状病毒肺炎(简称"新冠肺炎")患者的一线医务人员共计52名为火神山一线医务组,另选取同期非武汉地区某三甲综合医院重症病房工作的60名部队医务人员为非火神山一线医务组,对两组医务人员的疲劳情况、心理弹性及焦虑情况进行比较。结果两组医务人员疲劳情况中综合性疲劳、体力疲劳和活动减少以及心理弹性中坚韧性维度方面,差异有统计学意义(P<0.05)。在武汉支援时间不同、赶赴武汉支援前态度不同的火神山一线医务人员疲劳感得分比较,差异有统计学意义(P<0.05)。医生和护士之间、不同职称及自觉身体状况不同的火神山一线医务人员心理弹性得分比较,差异有统计学意义(P<0.05)。从业时间长短不同以及不同性别的火神山一线医务人员焦虑得分比较,差异有统计学意义(P<0.05)。结构方程模型结果显示:火神山一线医务人员的心理弹性直接负向影响其焦虑状态(β=-0.24,P<0.05),自觉疲劳情况直接正向影响其身体负荷(β=0.65,P<0.05)。结论在新冠肺炎疫情期间,心理弹性作为一个保护性变量,可通过激发个体自身内在潜力的作用,同时配合针对性的干预措施缓解医务人员焦虑和身体负荷,以达到保护身心健康的目的。
Objective To establish a physical and mental health model of front-line medical staff in Wuhan Huoshenshan Hospital during the epidemic period by using structural equation modeling(SEM).Methods A total of 52 first-line military medical workers who supported the treatment of COVID-19 patients in Wuhan Huoshenshan Hospital during February 17-29,2020 were selected as the Huoshenshan front-line medical group,and 60 military medical workers working in the intensive care unit of a Grade 3 Class A general hospital in non-Wuhan area during the same period were selected as the non-Huoshenshan front-line medical group.The status of fatigue,mental resilience and anxiety of the two groups were compared.Results There were statistically significant differences between the two groups in terms of comprehensive fatigue,physical fatigue and reduced activity,as well as the dimension of toughness in mental resilience(P<0.05).As to the Huoshenshan front-line medical group,there was significant difference in scores of fatigue of medical workers with different duration of support in Wuhan and different attitudes before going to Wuhan(P<0.05).There were statistically significant differences in the scores of mental resilience between doctors and nurses,and among medical workers with different professional titles and different perceived physical conditions(P<0.05).There were significant differences in scores of anxiety of medical staff with different length of medical service and gender(P<0.05).The results of structural equation modeling showed that mental resilience had a direct negative influence on anxiety state(β=-0.24,P<0.05),and selfperceived fatigue had a direct positive effect on body load(β=0.65,P<0.05).Conclusion tDuring the COVID-19 epidemic,mental resilience,as a protective variable,can stimulate the potential of he individual,and work with targeted interventions to alleviate the anxiety and physical burden of medical staff and protect their physical and mental health.
