基于PET影像组学预测非小细胞肺癌大分割放疗近期疗效 被引量:7

Prediction of Short-Term Effect for Non-Small Cell Lung Cancer Treated with Hypofractionated Radiotherapy Based on PET Radiomics

摘要 目的探讨基于PET影像组学对非小细胞肺癌(NSCLC)大分割放疗(HFRT)疗效的预测价值。资料与方法前瞻性研究30例经皮肺穿刺或经支气管镜取活检病理确诊HFRT患者,分别在放疗前后的PET图像上勾画31枚放疗靶病灶,即感兴趣区(ROI)。记录放疗前ROI基本参数,包括最大标准化摄取值(SUVmax)、平均标准化摄取值(SUVmean)、肿瘤代谢体积(MTV)、最大径(Dmax)等。放疗前后SUVmax变化率作为近期疗效评价指标。相关性分析筛选与近期疗效相关的基本参数建立预测模型1;采用LASSO法将影像组学特征降维后建立模型2;两套模型综合构建模型3。结果SUVmean及SUVmax与疗效呈正相关(r=0.613,P<0.001;r=0.499,P=0.004)。851项影像组学特征经LASSO降维后筛选出5项建立模型2。模型1和模型2的决定系数分别为:R^(2)=0.25,P=0.02;R^(2)=0.74,P<0.001。模型3与模型2决定系数一致(R^(2)=0.74)。结论基于PET影像组学模型可在HFRT前对NSCLC进行近期疗效预测,且其准确性高于基本参数模型。 Purpose To explore the predictive value of PET-based radiomics on the efficacy of hypofractionated radiotherapy(HFRT)for non-small cell lung cancer(NSCLC).Materials and Methods Thirty NSCLC treated with HFRT were prospectively enrolled in the study,31 radiotherapy target lesions of regions of interest(ROI)were drawn on PET images before and after radiotherapy.The basic parameters maximum standardized uptake value(SUVmax),mean standardized uptake value(SUVmean),metabolic tumor volume(MTV)and maximum diameter(Dmax)of ROI before radiotherapy were recorded.The change rate of SUVmax before and after radiotherapy was taken as the index of short-term curative effect evaluation.Correlation analysis was used to screen out the basic parameters related to the change rate of SUVmax,and established the basic parameter prediction model 1.LASSO regression was used to reduce the dimensionality to establish a linear prediction model 2.The two models were integrated to build model 3.Results SUVmean and SUVmax showed positive correlation with the curative effect(r=0.613,P<0.001;r=0.499,P=0.004).After dimension reduction by LASSO,5 features selected from 851 features were established model 2.The determination coefficients of model 1 and model 2 were:R^(2)=0.25,P=0.02;R^(2)=0.74,P<0.001;while the determination coefficients of model 3 and model 2 were consistent(R^(2)=0.74).Conclusion The PET-based radiomics model can predict the short-term efficacy of NSCLC before HFRT,and its accuracy is higher than that based on the basic parameter model.
