摘要
本文介绍临床药师参与1例激素依赖克罗恩病患者的诊疗过程,分析患者沙利度胺使用的合理性并小结药学监护要点。临床药师以免疫调节治疗方案的选择、给药剂量的调整及药品不良反应监测等作为切入点,配合医师,结合患者的实际情况制定个体化给药方案及监护计划,使患者的症状得到控制。临床药师在参与临床治疗过程中发挥药学专业优势,优化临床给药方案,减少药物不良反应,从而促进临床合理用药。
The paper introduced the clinical pharmacist’s participation in the diagnosis and treatment of a patient with corticosteroid addictive Crohn’s disease in cooperation with doctors and nurses,analyzed the rationality of thalidomide in patients with corticosteroid addictive Crohn’s disease and summarized the main points of pharmaceutical care information.With the consideration of the patient’s condition,clinical pharmacist provided pharmaceutical suggestions and performed pharmaceutical care by taking the selection of immunotherapy,dosage adjustment and pharmaceutical care of adverse drug reactions as the entry points.Finally,the patient’s symptom was under control.Clinical pharmacists should take advantage of professional pharmacy knowledge to participate in clinical treatment process to optimize medication therapy,reduce adverse drug reactions,and thus promote rational use of medicine in clinic.
