远隔缺血预处理方案对急诊开颅动脉瘤夹闭术患者神经功能的保护作用

The protective mechanism of remote ischemic preconditioning (RIPC) on the neurological function of patients undergoing emergency craniotomy for aneurysm clipping

摘要 目的探讨远隔缺血预处理(RIPC)方案对急诊开颅动脉瘤夹闭术患者神经功能的保护作用。方法选取我院收治的急诊开颅动脉瘤夹闭术患者100例,按照随机数字表法分为对照组和观察组,每组50例。观察组在气管插管后10 min于左上肢上臂采用测压袖带进行RIPC;对照组则将袖带绑于左上肢,不做处理。对比两组患者神经元特异性烯醇化酶(NSE)、中枢神经特异性蛋白(S100β)以及脑红蛋白(NGB)水平。结果两组患者T3、T4 NSE水平较T0降低,T2~T4 NGB水平较与T0升高,观察组T1~T4 S100β较T0降低,对照组T2~T4 S100β较T0降低(P<0.05);观察组T3、T4 NGB水平较对照组高(P<0.05);其他时间点两组NSE和S100β水平比较,差异无统计学意义(P>0.05)。结论RIPC方案可通过上调NGB水平以保护急诊开颅动脉瘤夹闭术患者的神经功能。 Objective To explore the he protective mechanism of remote ischemic preconditioning(RIPC)on the neurological function of patients undergoing emergency craniotomy for aneurysm clipping.Methods A total of 100 pa⁃tients undergoing emergency craniotomy for aneurysm clipping were selected and randomly divided into the control group and the observation group,50 cases in each group.In the observation group,a manometric cuff for RIPC was used on the left upper arm at 10 minutes after tracheal intubation,while in the control group a manometric cuff was tied to the left up⁃per limb without other treatment.The levels of neuron-specific enolase(NSE),central nervous system-specific protein(S100β)and neuroglobin(NGB)were compared between the two groups.Results The NSE levels of the two groups at T3 and T4 were lower than those at T0,and the NGB level at T2~T4 was higher than that at T0.The S100βlevel in the ob⁃servation group at T1~T4 was lower than that at T0,and the S100βlevel in the control group at T2~T4 was lower than that at T0(P<0.05).There was no significant difference in the levels of NSE and S100βbetween the two groups at other time points(P>0.05).Conclusion RIPC can increase the level of NGB to protect the nerve function of patients undergoing emergency craniotomy for aneurysm clipping.
