摘要
目的分析应用盐酸氨溴索联合纤维支气管镜肺泡灌洗方案治疗重症肺炎的效果。方法选取高唐县人民医院在2018年4月-2019年7月收治的98例重症肺炎患者开展研究,随机分为单一组和联合组各49例。单一组患者采用盐酸氨溴索治疗,联合组患者采用盐酸氨溴索联合纤维支气管镜肺泡灌洗方案治疗,比较两组患者最终治疗效果、炎症介质水平、呼吸机带机时间、住院时间及治疗后1周急性生理与慢性健康评分(APACHⅡ)。结果联合组总有效率为97.96%,高于单一组的81.63%;联合组患者接受治疗后CRP、IL-6、IL-8等外周血清炎症介质水平均低于单一组;联合组患者接受治疗后呼吸机带机时间、住院时间均短于单一组,APACHEⅡ评分低于单一组,差异均有统计学意义(P<0.05)。结论应用盐酸氨溴索联合纤维支气管镜肺泡灌洗方案治疗重症肺炎的效果明显,能有效减轻患者肺部炎症,预后较好,值得在临床中应用推广。
Objective To analyze the effect of ambroxol hydrochloride combined with bronchofiberscope alveolar lavage plan in the treatment of severe pneumonia.Methods From April 2018 to July 2019,98 patients with severe pneumonia in Gaotang County People’s Hospital were selected for the study and randomly divided into single group and combined group,with 49 cases in each group.The patients in the single group were treated with ambroxol hydrochloride,and the patients in the combined group were treated with ambroxol hydrochloride combined with bronchofiberscope alveolar lavage plan.The final treatment effect,the levels of inflammatory mediators,administration time of ventilator,hospitalization time and acute physiology and chronic health valuation(APACHE Ⅱ) after 1 week of treatment of the two groups were compared.Results The total effective rate of the combined group was 97.96%,which was higher than81.63% of the single group;after treatment,the levels of CRP,IL-6,IL-8 and other inflammatory mediators in the combined group were lower than those in the single group;after treatment,the adi mini strati on time of ventilator and hospitalization time in the combined group were shorter than those in the single group,and the APACHE Ⅱ score was lower than single group,and the differences were statistically significant(P <0.05).Conclusion Application of ambroxol hydrochloride combined with bronchofiberscope alveolar lavage plan in the treatment of severe pneumonia had obvious effect,could effectively reduce the lung inflammation of patients and the prognosis was good,which was worthy of clinical application and promotion.
