摘要
目的:分析破裂前交通动脉瘤运用眶上外侧入路显微夹闭术治疗的临床效果。方法:选择信阳市中心医院2016年10月—2019年10月接收的64例破裂前交通动脉瘤患者,根据抽签法将其分为观察组(n=32)和对照组(n=32)。两组患者均行显微夹闭术,对照组经翼点入路,观察组经眶上外侧入路。比较两组围术期指标、预后优良率及术后并发症发生率。结果:与对照组相比,观察组手术总用时短,切口长度、术中失血量及骨瓣大小均较少,差异均有统计学意义(P<0.05)。两组预后优良率比较,差异不显著(P>0.05)。两组慢性脑积水发生率比较,差异不显著(P>0.05);观察组颅内感染、头皮愈合不良发生率较对照组低,差异均有统计学意义(P<0.05)。结论:眶上外侧入路显微夹闭术和翼点入路显微夹闭术均可有效治疗破裂前交通动脉瘤,而眶上外侧入路具有创伤小、术后并发症少等优点。
Objective:To analyze the clinical effect of treatment of ruptured anterior communicating artery aneurysms with microsopic clipping lateral approach.Methods:64 patients with ruptured anterior communicating artery aneurysms in the hospital from October 2016 to October 2019 were selected and divided into observation group(n=32) and control group(n=32) according to the drawing method.Both groups underwent microscopic clipping,the control group received a pterional approach,and the observation group received a superior orbital approach.The perioperative indexes,the excellent and excellent rate of prognosis and the incidence of postoperative complications were compared between the two groups.Results:Compared with the control group,the total operation time of the observation group was shorter,the incision length,intraoperative blood loss and bone flap size were less,the differences were statistically significant(P<0.05).There was no significant difference in the excellent rate between the two groups(P>0.05).There was no significant difference in the incidence of chronic hydrocephalus between the two groups(P>0.05).The incidence of intracranial infection and poor scalp healing in the observation group was lower than that in the control group(P<0.05).Conclusion:Both lateral supraorbital approach and pterional approach are effective in the treatment of ruptured anterior communicating artery aneunysms.
