摘要
新型冠状病毒感染疾病(corona virus disease 2019,COVID-19)已成为全球流行性疾病,中医药在防治中发挥了重要的作用,因此分析本病病机演变和恶变的发展规律,对于指导其及早干预、防止向重症进展和常态化防治具有重要意义。本文基于中医药理论,分析和明确了COVID-19属于寒疫和湿疫夹杂的混合疫,为外寒湿引动内湿伏邪所致;系统分析和探讨中医病机演变的动态规律和病机恶变的驱动因素,提出了病机由传变-剧变-恶变-极变-转归动态发展规律,提出以寒湿疫疠直中三阴的六经辨证结合三焦辨证为主要辨证论治的思路,并归纳了中医辨证施药的思路和治疗禁忌,指出"截断病势"和"防止脏寒"疗法在常态化治疗和预后中的关键地位。
Corona virus disease 2019(COVID-19) has become a global pandemic disease. Traditional Chinese medicine(TCM) plays an important role in prevention and treatment. Therefore, it is of great significance to analyze rules of its pathological evolution and deterioration for early intervention and prevention of its progression to severe disease. Based on TCM theory, COVID-19 was analyzed to be a mixed epidemic of cold and dampness epidemics, which is caused by external cold-dampness stirring internal hidden pathogenic dampness. The dynamic rules of the pathological evolution and driving forces of the pathological deterioration in TCM were systematically analyzed, and a pathological dynamic developmental rule of transmission-upheaval-deterioration-extreme change-rehabilitation was proposed. Therefore, the main idea was put forward in syndrome differentiation and treatment, combining syndrome differentiation of six channels theory based on direct attack of the cold-dampness epidemic on three yin channels with syndrome differentiation of sanjiao theory. Ideas on TCM syndrome differentiation and medication as well as contraindications in the treatment were summarized, pointing out the key roles of"truncating progression"and"preventing cold zang-viscera"therapies in the treatment and prognosis.
