摘要
目的分析经阴道超声诊断卵巢囊肿蒂扭转的应用效果及其影像学表现。方法 73例疑似卵巢囊肿蒂扭转患者为研究对象,均应用经腹部和经阴道超声检查。以手术或腹腔镜探查结果为金标准,比较两种检查方法的准确度、敏感度、特异度,误诊、漏诊情况;观察两种检查方法的卵巢囊肿蒂扭转影像学表现。结果以手术或腹腔镜探查结果为金标准, 73例患者中有70例确诊为卵巢囊肿蒂扭转。经阴道超声检查的准确度为94.52%(69/73)、敏感度为95.71%(67/70)、特异度为66.67%(2/3),经腹部超声检查的准确度为76.71%(56/73)、敏感度为78.57%(55/70)、特异度为33.33%(1/3)。经阴道超声检查的准确度、敏感度均高于经腹部超声检查,差异具有统计学意义(P<0.05)。两种检查方法的特异度比较,差异无统计学意义(P>0.05)。经阴道超声检查的误漏诊率为5.48%(4/73),低于经腹部超声检查的23.29%(17/73),差异具有统计学意义(χ^(2)=9.400, P<0.05)。经阴道超声检查在囊肿血流、厚度、回声等影像学特征方面表现更好。结论经阴道超声检查的诊断效果优于经腹部超声,其可为卵巢囊肿蒂扭转的诊断提供准确的影像学信息。
Objective To analyze the practical effect and imaging manifestations of transvaginal ultrasonography in diagnosis of torsion of ovarian cyst pedicle.Methods 73 patients with suspected torsion of ovarian cyst pedicle as the study subjects all were examined by transabdominal and transvaginal ultrasonography.Taking the results of surgery or laparoscopic exploration as the gold standard,the accuracy,sensitivity,specificity,misdiagnosis and missed diagnosis of the two inspection methods were compared,and the imaging manifestations of the ovarian cyst pedicle torsion of the two inspection methods.Results Taking the results of surgery or laparoscopic exploration as the gold standard,70 cases of the 73 patients were diagnosed with torsion of ovarian cyst pedicle.The accuracy,sensitivity and specificity of transvaginal ultrasonography were 94.52%(69/73),95.71%(67/70)and 66.67%(2/3).The accuracy,sensitivity and specificity of transabdominal ultrasonography were 76.71%(56/73),78.57%(55/70)and 33.33%(1/3).The accuracy and sensitivity of transvaginal ultrasonography were higher than that of transabdominal ultrasonography,and the difference was statistically significant(P<0.05).There was no statistically significant difference in specificity between the two methods(P>0.05).The rate of misdiagnosis and missed diagnosis of transvaginal ultrasonography was 5.48%(4/73),which was lower than 23.29%(17/73)of transabdominal ultrasonography,and the difference was statistically significant(χ2=9.400,P<0.05).Transvaginal ultrasonography was better in the imaging features of cyst blood flow,thickness,and echo.Conclusion The diagnostic effect of transvaginal ultrasonography is better than that of transabdominal ultrasound,which can provide accurate imaging information for the diagnosis of torsion of ovarian cyst pedicle.
