摘要
目的构建基于立体MDT模式引导下2型糖尿病(T2DM)患者临床护理路径,旨在使临床护理工作流程化、路径化。方法以住院患者需求调查表与患者舒适度调查表为横断面调查工具,对2018年1月~11月在我院内分泌科接受住院治疗的80例T2DM患者的护理需求、满意度及舒适状况进行横断面调查,为基于立体MDT模式引导下T2DM患者临床护理路径的构建提供实践依据。采用随机数字表法将2019年1月~11月在我院内分泌科接受住院治疗的120例T2DM患者分为对照组和试验组(每组60例)进行临床护理路径实证研究。结果横断面调查显示,患者入院时对心理社会支持护理需求最高(62.31%),住院期间对疾病管理护理需求最高(62.60%),出院前1 d对健康教育护理需求最高(66.34%);入院时、住院期间、出院前1 d心理社会支持满意度均为最低(分别为64.82%、63.72%和59.84%);患者心理精神舒适度最差(13.8%)。试验组患者舒适状况(环境除外)优于对照组(P<0.05);试验组患者工作态度、健康教育、心理指导的满意度得分及总分均高于对照组(P<0.05)。结论基于立体MDT引导下T2DM患者临床护理路径的构建与应用,利于临床护理资源的合理配置,进一步升华优质护理内涵,促进整体护理向更深层次发展,将患者引向更好的康复结局。
Objective To construct a nursing path for T2DM patients under the guidance of the stereo MDT mode,so that the nursing work can be streamlined and routed.Methods Using inpatient needs questionnaire and patient comfort questionnaire as cross-sectional survey tools,the nursing needs,satisfaction and comfort status of 80 patients with T2DM who were hospitalized in the Department of endocrinology of our hospital from January to November 2018 were cross-sectional investigated,so as to provide practical basis for the construction of clinical nursing pathway for patients with T2DM under the guidance of three-dimensional MDT mode.120 T2DM patients who were hospitalized in the Department of endocrinology of our hospital from January to November 2019 were randomly divided into control group and experimental group(60 cases in each group).Results The cross-sectional survey showed that the patients had the highest demand for psychosocial support(62.31%)at admission,disease management(62.60%)during hospitalization and health education(66.34%)one day before discharge.The satisfaction of psychosocial support was the lowest at admission,during hospitalization and one day before discharge(64.82%,63.72%and 59.84%respectively).The mental comfort of patients was the worst(13.8%).The comfortable condition(except environment)of the experimental group was better than that of the control group(P<0.05).The satisfaction scores and total scores of work attitude,health education and psychological guidance in the experimental group were higher than those in the control group(P<0.05).Conclusion The construction and application of clinical nursing pathway for patients with T2DM under the guidance of three-dimensional MDT is conducive to the rational allocation of clinical nursing resources.To further sublimate the connotation of high-quality nursing and promote the development of holistic nursing to a deeper level will lead patients to a better rehabilitation outcome.
