耳前颞下经岩前入路切除岩斜区肿瘤 被引量:1

Resection of petroclival tumors through the anterior-subtemporal transanterior petrosal approach

摘要 目的探讨耳前颞下经岩前入路切除岩斜区肿瘤的疗效。方法回顾性分析58例岩斜区肿瘤病例资料,均采用耳前颞下经岩前入路手术切除。结果肿瘤全切除46例,近全切除5例,大部切除7例。并发症:复视23例,面部麻木20例。术后随访6~63个月,平均30个月。术后伽玛刀治疗6例,肿瘤均未发生进展。2例转移瘤病人因肿瘤复发而死亡。结论耳前颞下经岩前入路适用于切除岩斜区肿瘤,该入路对头皮、颞肌、血管和神经的损伤小,临床可作为一种良好选择。 Objective To explore the efficacy of the anterior-subtemporal transanterior petrosal approach in the resection of petroclival tumors. Methods The clinical data of 58 patients with petroclival tumors were analyzed retrospectively. The tumors were surgically resected through anterior-subtemporal transanterior petrosal approach. Results The total tumor resection was achieved in 46 cases,subtotal resection in 5, major resection in 7. Complications were diplopia in 23 cases and facial numbness in 20. The follow-up period was 6-63 months, with an average of 30 months. Six cases were treated with Gamma Knife after operation, none of the tumors progressed. Two patients with metastatic tumors died to tumor recurrence. Conclusions The anterior-subtemporal transanterior petrosal approach is suitable for the resection of petroclival tumor. This approach has little damage to the scalp, temporal muscles,blood vessels and nerves, and can be used as a good clinical choice.
