摘要
目的:观察心肌肌钙蛋白(cTnI)、肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)及N端脑钠肽前体(NT-proBNP)水平检测在甲状腺功能亢进(甲亢)性心脏病诊断中的效能。方法:选取2017年6月至2019年5月收治的甲亢性心脏病患者71例为观察组,另选取同期年龄、性别比例匹配的单纯甲亢患者55例为对照组,检测并比较两组心肌酶谱标志物[cTnI、CK、CK-MB]与NT-proBNP水平及心功能指标水平。结果:观察组血清cTnI、CK、CK-MB及NT-proBNP水平均高于对照组,左心室射血分数(LVEF)、左心室舒张末期内径(LVDD)及E/A值均低于对照组,差异有统计学意义(P<0.05);血清cTnI、CK、CK-MB及NT-proBNP联合检测曲线下面积(AUC)为0.964,95%CI为0.933~0.995,对于甲亢性心脏病患者的鉴别能力优于单项检测。结论:甲亢性心脏病患者血清cTnI、CK、CK-MB及NT-proBNP水平呈异常升高,联合检测甲亢性心脏病的诊断效能高于单项检测诊断效能。
Objective:To investigate efficacies of detection of cardiac troponin(cTnI),creatine kinase(CK),creatine kinase isoenzyme(CK-MB),and N-terminal pro-brain natriuretic peptide(NT-proBNP)levels in diagnosis of hyperthyroid heart disease.Methods:71 patients with hyperthyroid heart disease admitted from June 2017 to May 2019 were selected as observation group,and 55 patients only with hyperthyroidism with matched age and sex ratio in the same period were used as control group.The myocardial enzyme spectrum marker[cTnI,CK,CK-MB]levels and NT-proBNP levels and cardiac function index levels were detected and compared between the two groups.Results:The serum cTnI,CK,CK-MB and NT-proBNP levels in the observation group were higher than those in the control group;the LVEF,LVDD and E/A values were lower than those in the control group;and the differences were statistically significant(P<0.05).The combined detection of CK-MB and NT-proBNP had an AUC of 0.964,95%CI of 0.933-0.995,which was better than the single test in the identification of the patients with hyperthyroid heart disease.Conclusions:The levels of serum cTnI,CK,CK-MB and NT-proBNP in the patients with hyperthyroid heart disease are abnormally elevated.The diagnostic efficiency of the combined detection of hyperthyroid heart disease is higher than that of single detection.
