摘要
目的调查中老年女性阴道炎患者阴道微生态状况。方法采集938例中老年女性阴道炎患者阴道分泌物,通过盐水湿片检查、菌群革兰染色及干化学分析,综合判定阴道微生态状况。结果938例患者中微生态失调902例,占96.16%,其中单纯性感染342例(37.92%,342/902)中需氧菌性阴道病(17.85%,161/902)检出率最高,混合感染172例(19.07%,172/902)中需氧菌性阴道病合并细菌性阴道病(8.31%,75/902)检出率最高。外阴阴道假丝酵母菌病、混合感染、过氧化氢(H 2O 2)阳性、唾液酸苷酶阳性检出率随年龄增长下降,需氧菌性阴道病、优势菌抑制、pH>4.5、白细胞酯酶阳性检出率随年龄增长上升,差异有统计学意义(P<0.05)。结论中老年女性阴道炎患者阴道微生态失调率高,随年龄变化阴道微生物菌群结构及阴道炎发病率呈现不同特点。
Objective To investigate the vaginal microecology of the middle-aged and elderly women with vaginitis.Methods Vaginal secretions were collected from 938 middle-aged and elderly women with vaginitis,and the microecological status of vagina was comprehensively determined by saline wet tablet,gram staining of flora and dry chemistry analysis.Results Among the 938 patients,902 cases(96.16%)had microecological disorder.Among the 342 cases with simple infection(37.92%,342/902),aerobevaginosis had the highest detection rate(17.85%,161/902).Among the 172 cases with mixed infection(19.07%,172/902),aerobevaginosis associated with bacterial vaginosis had the highest detection rate(8.31%,75/902).The detection rate of vulvovaginal candidiasis,mixed infection,positive hydrogen peroxide(H 2O 2)and positive sialidase decreased with the increase of age,while the detection rates of aerobevaginosis,dominant bacteria inhibition,pH>4.5,and positive leukocyte esterase increased with the increase of age,and the differences were statistically significant(P<0.05).Conclusion Vaginal microecological imbalance rate is high among middle-aged and elderly women with vaginitis.The vaginal microflora structure and vaginitis incidence rate vary with age.
