摘要
目的探讨自拟清肺化痰汤联合超短波治疗脑卒中合并肺部感染的临床效果。方法选取2018年11月—2019年11月治疗的脑卒中合并肺部感染患者80例,随机数字表法分为对照组(40例)和观察组(40例)。对照组采用常规抗感染、化痰、维持水电解质平衡以及脑卒中常规西药治疗,包括营养脑神经、改善脑代谢、控制血压血糖及对症支持处理;观察组在对照组基础上给予自拟清肺化痰汤联合超短波治疗。治疗后对患者效果进行评估,比较患者的生命体征、CRP含量、白细胞计数、排痰量、脑卒中合并肺部感染患者可导致并发症的概率。结果 2组患者的体温、呼吸、心率、NBP、SBP等情况比较,符合正态性分布的用t检验,不符合正态性分布的用非参数检验,差异有统计学意义(P <0. 05);治疗后,2组感染患者的CRP含量、白细胞计数、排痰量均低于治疗前,且观察组均低于对照组(P <0. 05);脑卒中合并肺部感染患者中,大于80岁的老年患者并发肾功能不全、心力衰竭、电解质紊乱的概率相对较大,差异具有统计学意义(P <0. 05)。结论清肺化痰汤联合超短波治疗脑卒中合并肺部感染的临床效果显著,有利于减轻患者痛苦,降低并发症发生概率,值得推广应用。
Objective To investigate the clinical study of self-made Qingfei Huayuan decoction combined with ultra-short wave in the treatment of stroke with pulmonary infection. Methods According to 80 patients with pulmonary infection who were treated in our hospital from November 2018 to November 2019,they were divided into control group( n = 40) and observation group( n = 40) by the randomized digital table. The control group was treated with Routine anti infection,expectoration,maintaining water and electrolyte balance,and routine western medicine treatment for stroke,including nutrition of brain nerve,improvement of brain metabolism,control of blood pressure and blood glucose and symptomatic support treatment,and the observation group was treated with Qingfei Huatan decoction and ultra-short wave radio waves on the basis of the control group. The patient’s effect was evaluated,and the patient’s vital signs,CRP content,white blood cell count,sputum discharge,stroke with pulmonary infection patients can lead to complications probability was compared. Results Compared with the vital signs,body temperature,respiration,heart rate,NBP,SBP and so on,the t test was used to accord with the normal distribution,and the nonparametric test was not consistent with the normal distribution,the difference was not statistically significant( P <0. 05). After treatment,the CRP content,white blood cell count and sputum discharge of the two groups were lower than those of the posttreatment,and the values of the observation group were lower than those of the control group( P < 0. 05). Comparison of the probability of complications in patients with stroke and lung infection,elderly patients older than 80 years old are relatively more likely to develop renal insufficiency,heart failure,and electrolyte imbalance( P < 0. 05). Conclusion The clinical effect of Qingfei Huatan decoction combined with ultra-short wave in stroke complicated with pulmonary infection is remarkable,which is beneficial to alleviate the pain of patients and reduce the probability of complications. It is worth popularizing and applying.
