摘要
目的探讨闭合复位髓内钉内固定对老年肱骨干骨折患者的治疗效果。方法选取2018年9月~2019年8月在牡丹江林业中心医院骨科就诊的80例老年肱骨干骨折患者作为研究对象。采用随机数字表法,将患者分为切开组和闭合组,每组患者各40例。切开组行切开复位钢板螺钉内固定治疗,闭合组行闭合复位髓内钉内固定治疗。比较两组的术中出血量、手术时间、住院时间、肱骨干骨折愈合时间、肘关节功能恢复情况及并发症发生率。结果闭合组术中出血量少于切开组,手术时间和住院时间均短于切开组,肘关节功能评分优良率高于切开组,并发症总发生率低于切开组,差异均有统计学意义(P<0.05)。两组患者在骨折愈合时间方面比较,差异无统计学意义(P>0.05)。结论闭合复位髓内钉内固定术可有效提高老年肱骨干骨折患者的治疗效果。
Objective To investigate the curative effect of closed reduction and intramedullary nail internal fixation in elderly patients with humeral shaft fracture.Methods A total of 80 elderly patients with humeral shaft fracture admitted to Mudanjiang Forestry Central Hospital from September 2018 to August 2019 were selected as the study subjects.The patients were divided into the open group(40 cases)and the closed group(40 cases)according to the random number table method.In the open group,open reduction and internal fixation with plate and screw were performed.In the closed group,closed reduction and internal fixation with intramedullary nail were performed.Intraoperative blood loss,operative time,hospital stay,humeral shaft fracture healing time,elbow joint functional recovery and complication rate were compared between the two groups.Results The intraoperative blood loss of patients in the closed group was less than that in the open group,the operative time and hospital stay were shorter than those in the open group,the excellent and good rate of elbow joint function score was higher than that in the open group,and the total incidence of complications was lower than that in the open group,and the differences were statistically significant(P<0.05).There was no significant difference in fracture healing time between the two groups(P>0.05).Conclusion Closed reduction and internal fixation with intramedullary nail can effectively improve the treatment of humeral shaft fracture in the elderly.
