摘要
目的探讨慢性心力衰竭患者采用坎地沙坦联合螺内酯治疗的临床效果及对患者心功能、血流动力学的影响。方法选取2019年1—12月于该院接受治疗确诊为慢性心力衰竭的患者83例作为研究对象,根据患者入院编号以单双号法分为两组,对照组单纯采用螺内酯治疗,观察组在其基础上联合坎地沙坦,分析临床治疗效果,检测对比两组患者治疗前后心功能指标变化及血流动力学差异。结果观察组患者用药后临床疗效明显优于对照组(97.62%vs 80.49%)(P<0.05);两组患者未接受治疗前心功能、血流动力学相应指标差异无统计学意义(P>0.05),用药治疗后均明显改善,观察组患者心功能指标LVEDD(55.01±1.21)mm、EF(48.72±2.03)%明显高于对照组,心率(82.23±5.35)次/min低于对照组(t=10.130、15.920、11.790,P<0.05),同时血流动力学指标血氧饱和度、动脉氧分压、氧合指数均高于对照组(P<0.05)。结论慢性心力衰竭患者采用坎地沙坦联合螺内酯效果确切,能够有效改善患者心功能级血流动力学,安全性佳。
Objective To investigate the clinical effect of candesartan combined with spironolactone in patients with chronic heart failure and its influence on the cardiac function and hemodynamics.Methods A total of 83 patients diagnosed with chronic heart failure who received treatment in the hospital from January to December 2019 were selected as the research objects.The patients were divided into two groups according to the admission number of the patients and the odd and even numbers,the control group was treated with spironolactone alone,and the observation group was combined with candesartan on the basis of it to analyze the clinical treatment effect,and to detect and compare the changes in cardiac function indexes and hemodynamic differences between the two groups before and after treatment.Results The clinical efficacy of the observation group was significantly better than that of the control group(97.62%vs 80.49%)(P<0.05);there was no statistically significant difference in the corresponding indexes of cardiac function and hemodynamics between the two groups of patients before treatment(P>0.05),and they were significantly improved after treatment.The cardiac function indexes of the observation group were LVEDD(55.01±1.21)mm and EF(48.72±2.03)%,which was significantly higher than the control group,and the heart rate(82.23±5.35)beats/min was lower than that of the control group(t=10.130,15.920,11.790,P<0.05).At the same time,hemodynamic indicators,blood oxygen saturation,arterial oxygen partial pressure and oxygenation index were higher than those of the control group(P<0.05).Conclusion The use of candesartan combined with spironolactone in patients with chronic heart failure has a definite effect,which can effectively improve the hemodynamics of the patients'cardiac function level,with good safety.
