摘要
目的研究宫腔镜电切术联合左炔诺孕酮宫内缓释系统(LNG-IUS)治疗子宫内膜息肉疗效及对雌激素受体(ER)和孕激素受体(PR)表达的影响。方法选取2016年3月至2017年7月四川省广元市中医医院收治的118例子宫内膜息肉患者作为研究对象。随机将其分为研究组(n=59)和对照组(n=59)。对照组患者行宫腔镜电切术,在此基础上,研究组患者加施LNG-IUS治疗。两组患者术后均行常规抗感染治疗。比较两组患者治疗前、治疗后月经失血图(PBAC)评分、生殖激素水平、子宫内膜厚度、血红蛋白、不良反应、复发情况、ER及PR表达水平。结果治疗后3个月、6个月、12个月和18个月,研究组患者PBAC评分、子宫内膜厚度、不良反应发生率及复发率均明显低于对照组患者,差异均具有统计学意义(均P<0.05),而血红蛋白水平则明显高于对照组患者,差异具有统计学意义(P<0.05);研究组患者ER、PR表达出现明显下调,差异具有统计学意义(P<0.05),而对照组患者ER、PR的表达差异无统计学意义(P>0.05);此外,两组患者生殖激素水平未见明显差异,差异无统计学意义(P>0.05)。结论宫腔镜电切术联合LNG-IUS治疗子宫内膜息肉疗效显著,可有效调节ER、PR的表达,安全可靠且不易复发,值得临床推荐。
Objective To study the effect of hysteroscopic resection combined with levonorgestrel intrauterine sustained release system(LNG-IUS)in the treatment of endometrial polyps and the effect on the expression of estrogen receptor(ER)and progesterone receptor(PR).Methods 118 patients with endometrial polyps admitted in Guangyuan Hospital of Traditional Chinese Medicine from March 2016 to July 2017 were selected and divided into control group and study group by random number method,each of 59 cases.The control group was treated with hysteroscopic resection.On its basis,the study group was treated with LNG-IUS.Both the two groups were treated with routine anti-infective therapy.The levels of PBAC scores,reproductive hormone levels,endometrial thickness,hemoglobin,adverse reaction,recurrence,ER and PR were compared between the two groups before and after treatment.Results At 3,6,12 and 18 months after treatment,PBAC scores,endometrial thickness,adverse reaction rate and recurrence rate were significantly lower in the study group than those in the control group and the hemoglobin level was significantly higher,all with statistically significant differences(all P<0.05).The expression of ER and PR in the study group was significantly decreased(P<0.05),but the expression of ER and PR in the control group did not change significantly(P>0.05).In addition,there was no significant difference in reproductive hormone levels between the two groups(P>0.05).Conclusions Hysteroscopic resection combined with LNG-IUS is effective in the treatment of endometrial polyps,which can effectively regulate the expression of ER,PR,safe and reliable and difficult to relapse,thusly worthy of clinical recommendation.
