摘要
目的:比较椎管内麻醉与全身麻醉两种方式对老年患者下肢骨折手术预后的影响。方法:将2017~2019年收治下肢骨折需行手术治疗的老年患者184例按麻醉方式不同分为两组,A组椎管内麻醉126例,B组全身麻醉58例,利用统计学方法分析两种麻醉方式对患者预后的影响。结果:椎管内麻醉组T 1~T 6均显著高于全麻组,差异有统计学意义(P<0.05);术后椎管内麻醉组转入重症监护室64例,转入普通病房62例;全身麻醉组转入重症监护室58例,转入普通病房0例,差异有统计学意义(P<0.05);术后椎管内麻醉组平均住院时长明显低于全身麻醉组,差异有统计学意义(P<0.05);两组术后并发症比较,差异无统计学意义(P>0.05)。结论:老年下肢手术患者采取椎管内麻醉和全身麻醉的术后病死率和并发症发生率差异无统计学意义。但基层医院麻醉条件限制,全身麻醉不仅增加患者经济负担,而且增加患者平均住院时间。
Objective:To compare the effects of intraspinal anesthesia and general anesthesia on the prognosis of lower extremity fractures in elderly patients.Methods:A total of 184 elderly patients with lower limb fractures requiring surgical treatment admitted to Kaifeng Central Hospital from 2017 to 2019 were selected and divided into two groups according to different anesthesia methods:126 patients in group A and 58 patients in group B underwent general anesthesia.The effects of the two anesthesia methods on the prognosis of patients were compared and analyzed.Results:T 1~T 6 in the spinal anesthesia group was significantly higher than that in the general anesthesia group,with statistically significant difference(P<0.05).After surgery,64 cases in the spinal anesthesia group were transferred to intensive care unit,62 cases to general ward,and 58 cases in the general anesthesia group were transferred to intensive care unit.The difference between the two groups was statistically significant(P<0.05).The average length of stay in the spinal anesthesia group was significantly lower than that in the general anesthesia group,and the difference was statistically significant(P<0.05).There was no significant difference in postoperative complications between the two groups(P>0.05).Conclusion:There was no statistically significant difference in the postoperative mortality and complication rate between the elderly patients undergoing lower extremity surgery under intraspinal anesthesia and general anesthesia.However,due to the limitation of anesthesia conditions in primary hospitals,general anesthesia not only increases the economic burden of patients,but also increases the average length of hospital stay.
