摘要
目的探讨叙事疗法对脑卒中主要照顾者的焦虑抑郁情绪、照顾负担以及益处发现水平的影响。方法应用随机数字表法将某三级甲等医院康复科和神经内科的64例脑卒中主要照顾者分为试验组(35例)和对照组(29例),对照组患者实施常规护理,试验组患者在此基础上实施叙事疗法干预;干预前后采用焦虑抑郁自评量表、照顾者负担量表和脑卒中照顾者获益感问卷评价效果。结果经过3个月的干预,随着时间的推移,干预后不同时间点试验组脑卒中主要照顾者焦虑及抑郁评分明显低于对照组(均P<0.05),而照顾负担得分组间比较,差异无统计学意义(P>0.05);随着时间的推移,干预后不同时间点试验组脑卒中主要照顾者益处发现总分、个人成长维度得分及自我获取维度得分明显高于对照组(均P<0.05),而健康促进维度得分、家庭成长维度得分组间比较,差异无统计学意义(均P>0.05)。结论叙事疗法能够改善脑卒中主要照顾者的焦虑抑郁情绪,可有效提高其益处发现水平。
Objective To explore the effect of narrative therapy on the anxiety and depression,the burden of care and the level of benefit discovery of the main caregivers of stroke patients.Methods 64 main caregivers of stroke patients were randomly divided using a random digit table into trial group(n=35)and control group(n=29).The control group was given routine nursing,and the trial group was given narrative therapy on the basis of routine nursing.The effects before and after intervention were evaluated by self-rating anxiety scale,self-rating depression scale,caregiver burden scale and stroke caregiver'benefit-finding questionnaire.Results After 3 months of intervention,the scores of anxiety and depression of the main caregivers in the trial group at different time points after intervention were significantly lower than those in the control group(P<0.05),but there was no statistical difference in the score of care burden score between the two groups(P>0.05).As time passed by,the total score of benefit finding,personal growth dimension score and self acquisition dimension score of the main caregivers in the trial group were significantly higher than those in the control group at different time points after intervention(P<0.05),but there was no significant difference in the scores on health promotion dimension score and family growth dimension between the two groups(P>0.05).Conclusion Narrative therapy can significantly alleviate the anxiety and depression of the main caregivers of stroke patients,and can effectively improve the level of benefit finding.
