摘要
目的探讨计算机断层扫描(CT)联合超声引导下经皮微波消融术在尾状叶肝癌患者中的应用价值及安全性。方法收集2015年11月至2018年2月在我院进行CT联合超声引导下经皮微波消融术的30例尾状叶肝癌患者,观察围手术期情况、疗效以及并发症发生情况。结果30例患者均成功完成手术,对于肝内其他部位有病灶而且可以进行消融治疗的,同时实行消融治疗。术后进行CT及超声检查评估病灶,未发现肿瘤残留。患者手术时间为60~134(86.49±14.58)min。术中失血量为5~15(7.59±1.45)mL,主要是皮肤穿刺造成失血。患者住院时间为5~14(7.85±1.24)d。30例患者均完成12个月的随访,28例患者未发现复发,其余2例患者在术后2、4个月出现尾状叶病灶复发,再次进行消融治疗,后随访未再复发。30例患者中,1例出现迟发性气胸,7例发热,11例右上腹部疼痛,5例呕吐。结论CT联合超声引导下经皮微波消融术在尾状叶肝癌患者中应用效果较好,安全性较高。
Objective To evaluate the value and safety of computed tomogrophy(CT)combined with ultrasound-guided percutaneous microwave ablation in the patients with caudate lobe liver cancer.Methods Thirty patients undergoing CT combined with ultrasound-guided ablation from November 2015 to February 2018 in our hospital were enrolled.The perioperative indicators,efficacy and complications were observed.Results All the 30 patients successfully completed the operation.For other parts of the liver,the lesions could be treated with ablation treatment,the ablation treatment was performed.CT and ultrasound were used to evaluate the lesions after surgery,and no tumor residuals were found.The operation time of the patient were 60-134(86.49±14.58)min.The amount of blood loss from surgery were 5-15(7.59±1.45)mL,which was mainly caused by skin puncture.The hospital stay were 5-14(7.85±1.24)d.Thirty patients were followed up for 12 months,no recurrence was found in 28 patients;2 patients had recurrence of caudate lobe lesions at 2 and 4 months after surgery,the ablation was performed again and no recurrence was observed after follow-up.Of the 30 patients,1 patient had delayed pneumothorax,7 patients fever,11 patients right upper abdominal pain,and 5 patients vomiting.Conclusion CT combined with ultrasound-guided percutaneous microwave ablation has good value and safety in patients with caudate lobe liver cancer.
