3D打印技术辅助下微创手术治疗桡骨远端粉碎性骨折对腕关节功能及生活自理能力的影响观察 被引量:3

Effect of Minimally Invasive Surgery for Comminuted Distal Radius Fractures Assisted by 3D Printing Technology on Wrist Joint Function and Activities of Daily Living

摘要 目的:探究3D打印技术辅助下微创手术治疗桡骨远端粉碎性骨折对腕关节功能与生活自理能力的影响。方法:选取本院2017年1月-2019年6月收治的66例桡骨远端粉碎性骨折患者为研究对象,36例患者采用3D打印技术辅助下微创手术治疗为观察组,30例患者采用常规复位内固定治疗为对照组。比较两组手术指标、腕关节功能、并发症发生情况、生活自理能力(ADL)及生活质量差异。结果:观察组手术时间、骨折恢复时间均短于对照组(P<0.05),术中透视次数、术中出血量均显著少于对照组(P<0.05)。术后6个月,观察组腕关节掌屈、背伸、桡偏、尺偏、旋前、旋后的活动度及握力均高于对照组(P<0.05)。两组并发症发生情况比较,差异无统计学意义(P>0.05)。术后6个月,两组ADL评分与生活质量评分均较术前显著提高(P<0.05),且观察组均高于对照组(P<0.05)。结论:3D打印技术辅助下微创手术治疗桡骨远端粉碎性骨折术程短、创伤小,患者腕关节功能恢复良好,有助于改善其生活自理能力和生活质量。 Objective:To explore the effect of minimally invasive surgery for comminuted distal radius fractures assisted by 3D printing technology on wrist joint function and activities of daily living.Method:A total of 66 patients with comminuted distal radius fractures admitted to our hospital from January 2017 to June 2019 were selected as the study subjects,36 patients treated with 3D printing assisted minimally invasive surgery were selected as the observation group,30 patients treated with conventional reduction and internal fixation were selected as the control group.The differences in surgical indicators,wrist joint function,complications,activities of daily living ADL and quality of life were compared between two groups.Result:The operative time and fracture recovery time in the observation group were shorter than those in the control group(P<0.05),the number of intraoperative fluoroscopy and intraoperative blood loss were significantly less than those in the control group(P<0.05).At 6 months after surgery,the waist joint ranges of the activity and grip strength in wrist flexion,dorsiflexion,radial deviation,ulnar deviation,pronation and postrotation in the observation group were higher than those in the control group(P<0.05).There was no statistically significant difference in the incidence rates of complications between two groups(P>0.05).At 6 months after surgery,the ADL score and quality of life score in two groups were significantly higher than those of before surgery(P<0.05),and the observation group were higher than thos e of the control group(P<0.05).Conclusion:Minimally invasive surgery assisted by 3D printing technology for the treatment of comminuted distal radius fracture has a short surgical course and little trauma,the patient’s wrist function recovers well,which is conducive to improving ADL and quality of life.
