摘要
目的:探讨胎儿肺动脉多普勒测量参数对新生儿呼吸窘迫的预测价值。方法:选取2018年9月-2019年9月在本院住院并愿意行胎儿肺动脉多普勒检查的孕妇100例于分娩前3 d进行研究,以新生儿结局情况为依据分为NRDS组(33例)及非NRDS组(67例),均采用GE LOGIC E9彩色多普勒超声诊断仪进行检查,观察不同孕周胎儿期主肺动脉内径(main pulmonary artery diamete,MPAD)及左右肺动脉内径(LPAD、RPAD),并对两组胎儿肺动脉不同多普勒取样位置收缩期加速时间(AT)/射血时间(ET)值情况进行对比,判断其预测效能。结果:胎儿主肺动脉及左右肺动脉内径随着孕周的增长而增加,三组间数据比较,差异有统计学意义(P<0.05)。NRDS组胎儿主肺动脉中段位置AT/ET比值较非NRDS组低,差异有统计学意义(P<0.05);但两组左(或右)肺动脉中段位置AT/ET比值比较,差异无统计学意义(P>0.05)。结论:胎儿主肺动脉内径(MPAD)、左右肺动脉内径(LPAD、RPAD)与肺动脉ET多普勒流速曲线AT/ET比值对新生儿呼吸窘迫的预测价值显著,可为临床诊断提供指导。
Objective:To explore the predictive value of fetal pulmonary artery Doppler measurement parameters for neonatal respiratory distress.Method:A total of 100 pregnant women who were hospitalized in this hospital from September 2018 to September 2019 and were willing to undergo fetal pulmonary artery Doppler examination were selected for the study 3 days before delivery.Based on the neonatal outcome,they were divided into NRDS group (33 cases) and non-NRDS group (67 cases).All of them were administered with GE LOGIC E9 color Doppler ultrasound diagnostic apparatus to observe the main pulmonary artery diameter (MPAD) and the left and right pulmonary artery diameters (LPAD,RPAD) of the fetal period in different gestational weeks.The systolic acceleration time (AT)/ejection time (ET) values of different Doppler sampling positions to judge its predictive performance were compared.Result:The inner diameters of the fetal main pulmonary artery and the left and right pulmonary arteries increased with the increase of gestational age.The data comparison between the three groups showed statistically significant differences (P<0.05).The ratio of AT/ET in the middle of the fetal main pulmonary artery in the NRDS group was lower than that in the non-NRDS group,the difference was statistically significant (P<0.05);but there was no significant difference in the ratio of AT/ET in the middle of the left (or right) pulmonary artery between the two groups (P>0.05).Conclusion:The AT/ET ratio of the fetal main pulmonary artery diameter,left and right pulmonary artery diameter,and pulmonary artery ET Doppler flow velocity curve has significant predictive value for neonatal respiratory distress,and can provide guidance for clinical diagnosis.
