摘要
目的探讨慢性丙型病毒性肝炎患者血小板水平与肝纤维化的相关性。方法选择2018年1月至2019年7月安阳市第五人民医院收治的39例慢性丙型病毒性肝炎患者为研究对象,根据血小板计数(BPC)将患者分为血小板减少(<100×10^9 L^-1)组(n=21)和血小板正常[(100~300)×10^9 L^-1]组(n=18),对2组患者的BPC及血清肝纤维化指标血清透明质酸(HA)、层粘连蛋白(LN)、Ⅲ型前胶原(PCⅢ)、Ⅳ型胶原蛋白(COLⅣ)水平进行比较,并分析血小板水平与血清HA、LN、PCⅢ和COLⅣ的相关性。结果血小板减少组患者BPC为(25~87)×10^9 L^-1,均值为(46.39±7.13)×10^9 L^-1;血小板正常组患者BPC为(112~137)×10^9 L^-1,均值为(123.27±10.12)×10^9 L^-1;血小板减少组患者BPC显著低于血小板正常组(t=9.041,P<0.05)。血小板减少组患者血清HA、LN、PCⅢ及COLⅣ水平均显著高于血小板正常组(t=13.512、8.527、8.062、12.619,P<0.05)。慢性丙型病毒性肝炎患者BPC与血清HA、LN、PCⅢ及COLⅣ水平均呈负相关(r=-0.491、-0.365、-0.427、-0.514,P<0.05)。结论慢性丙型病毒性肝炎患者血小板水平与肝纤维化有相关性,检测血小板水平有助于评估肝纤维化程度,为临床医师选择治疗方案及评估疾病预后提供帮助。
Objective To investi gate the correlation between platelet level and liver fibrosis in patients with chronic hepatitis C.Methods Thirty-nine patients with chronic hepatitis C admitted to the Fifth People′s Hospital of Anyang from January 2018 to July 2019 were selected as the research subjects,and the patients were divided into thrombocytopenia(<100×10^9 L^-1)group(n=21)and normal platelet(100×10^9-300×10^9 L^-1)group(n=18)according to the blood platelet count(BPC).The BPC and the levels of serum hyaluronic acid(HA),laminin(LN),typeⅢprecollagen(PCⅢ)and typeⅣcollagen(COLⅣ)were compared between the two groups,and the correlation between platelet level and serum HA,LN,PCⅢand COLⅣlevels was analyzed.Results In the thrombocytopenia group,the BPC was(25-87)×10^9 L^-1,with an average of(46.39±7.13)×10^9 L^-1.In the normal platelet group,the BPC was(112-137)×10^9 L^-1,with an average of(123.27±10.12)×10^9 L^-1.The BPC in the thrombocytopenia group was significantly lower than that in the normal platelet group(t=9.041,P<0.05).The levels of serum HA,LN,PCⅢand COLⅣin the thrombocytopenia group were significantly higher than those in the normal platelet group(t=13.512,8.527,8.062,12.619;P<0.05).The BPC was negatively correlated with serum HA,LN,PCⅢand COLⅣlevels in patients with chronic hepatitis C(r=-0.491,-0.365,-0.427,-0.514;P<0.05).Conclusion There is a correlation between platelet level and liver fibrosis in patients with chronic hepatitis C.Therefore,the detection of platelet level is helpful to evaluate the degree of liver fibrosis,and to help clinicians choose treatment options and evaluate the prognosis of the disease.
