摘要
目的探讨窄带成像内镜(NBI)下活检术在鼻咽癌早期诊断中的应用价值。方法选取2016年12月~2019年12月来我科就治的鼻咽部良性病变患者60例,其中30例行窄带成像内镜下活检术,另30例行常规鼻内镜下活检术。鼻咽癌早期患者50例,其中25例行窄带成像内镜下活检术,另25例行常规鼻内镜下活检术。110例患者均经病理确诊。比较两种方法的灵敏度、特异度、约登指数、阳性预测值等。结果两种检测方法所得检测结果基本特征比较:灵敏度、特异度、诊断符合率、约登指数分别为100.00%,84.00%;96.67%,93.33%;98.18%,89.09%;96.67%,77.33%。两者比较差异有统计学意义(P<0.05);NBI活检和鼻内镜活检阳性预测值为:96.00%,91.30%;NBI活检和鼻内镜活检阴性预测值为:96.67%,87.50%。两种检测方法阳性预测值差异有统计学意义(χ^2=4.010,P=0.046)。结论窄带成像内镜下活检术对鼻咽癌早期诊断具有较高的灵敏度、特异度、阳性预测值;具有快速、准确的优点,可用于鼻咽癌的筛检和早期诊断。
Objective To explore the application value of narrow-band imaging(NBI)endoscopic biopsy in early diagnosis of nasopharyngeal carcinoma(NPC).Methods 60 patients with benign nasopharyngeal lesions treated by our department from December 2016 to December 2019 were selected.Among them,30 patients underwent narrowband imaging endoscopic biopsy,and another 30 patients underwent conventional nasal endoscopic biopsy.Among 50 patients with early NPC,25 patients underwent narrow-band imaging endoscopic biopsy,and another 25 cases underwent conventional nasal endoscopic biopsy.All the 110 patients were confirmed by pathology.The sensitivity,specificity,Jordan index and positive predictive value of the two methods were compared.Results When the basic characteristics of the results of the two detection methods were compared,the sensitivity,specificity,diagnostic coincidence rate and Jordan index in pair were 100.00%,84.00%;96.67%,93.33%;98.18%,89.09%;96.67%,77.33%.The differences were statistically significant(P<0.05).The positive predictive value of NBI biopsy and nasal endoscopic biopsy was 96.00%and 91.30%respectively.The negative predictive value of NBI biopsy and nasal endoscopic biopsy was 96.67%and 87.50%respectively.The difference in positive predictive value between the two detection methods was statistically significant(χ^2=4.010,P=0.046).Conclusion NBI endoscopic biopsy has high sensitivity,specificity and positive predictive value for early diagnosis of NPC.With the advantages of rapidness and accuracy,it can be used for screening and early diagnosis of NPC.
