S100A8/A9、CS-846、MMP3在膝关节骨性关节炎中的表达及临床意义 被引量:12

Expression and clinical significance of S100A8/A9,CS-846 and MMP3 in knee osteoarthritis

摘要 目的探讨膝关节骨性关节炎患者血清钙卫蛋白(S100A8/A9)、硫酸软骨素846(CS-846)、基质金属蛋白酶3(MMP3)的表达水平及临床意义。方法选取2013年1月至2018年8月该科收治的膝关节骨性关节炎患者176例作为试验组,另选健康志愿者80例作为对照组。检测并比较两组受试者血清S100A8/A9、CS-846、MMP3水平,并比较不同Kellgren-Lawrence(K-L)分级膝关节骨性关节炎S100A8/A9、CS-846、MMP3的差异;采用Pearson分析S100A8/A9、CS-846、MMP3与K-L分级、西安大略麦克马斯特大学骨性关节炎调查量表(WOMAC)评分的相关性;应用受试者工作特征曲线(ROC曲线)分析S100A8/A9、CS-846、MMP3对膝关节骨性关节炎的预测及诊断价值。结果试验组患者的S100A8/A9、CS-846、MMP3水平分别高于对照组,差异有统计学意义(P<0.05);不同K-L分级的膝关节骨性关节炎患者S100A8/A9、CS-846、MMP3水平比较,差异有统计学意义(P<0.05)。Pearson相关性分析显示,血清S100A8/A9、CS-846、MMP3水平与K-L分级及WOMAC评分呈正相关(P<0.05)。ROC曲线分析结果显示,S100A8/A9、CS-846、MMP3联合检测曲线下面积为0.919,灵敏度和特异度分别为0.906和0.857。结论S100A8/A9、CS-846、MMP3在膝关节骨性关节炎患者表达水平升高,且与患者的病情严重程度有关;S100A8/A9、CS-846、MMP3水平的变化对膝关节骨性关节炎具有一定预测价值。 Objective To investigate the expression levels of serum calprotectin(S100A8/A9),chondroitin sulfate 846(CS-846)and matrix metalloproteinase 3(MMP3)and their clinical significance in patients with knee osteoarthritis.Methods From January 2013 to August 2018,176 patients with knee osteoarthritis treated in a department were selected as experimental group,while 80 healthy volunteers were selected as control group.The levels of serum S100A8/A9,CS-846 and MMP3 between the two groups were detected and compared.The differences of S100A8/A9,CS-846 and MMP3 in different K-L grading of knee osteoarthritis were compared.Pearson analysis was used to analyze the correlation between S100A8/A9,CS-846,MMP3 and K-L grading,WOMAC score.The predictive and diagnostic value of S100A8/A9,CS-846 and MMP3 for knee osteoarthritis was analyzed by receiver operating characteristic(ROC)curve.Results The levels of S100A8/A9,CS-846 and MMP3 in experimental group were higher than those in control group,and the difference was statistically significant(P<0.05).Patients with knee osteoarthritis of different K-L grading showed statistically significant differences in the levels of S100A8/A9,CS-846 and MMP3(P<0.05).Pearson correlation analysis showed that serum S100A8/A9,CS-846 and MMP3 levels were positively correlated with K-L grading and WOMAC score(P<0.05).The ROC curve results showed that the area under the curve of S100A8/A9,CS-846 and MMP3 combined detection was 0.919,and the sensitivity and specificity were 0.906 and 0.857,respectively.Conclusion The expression levels of S100A8/A9,CS-846 and MMP3 elevate in patients with knee osteoarthritis which are related to the severity of the disease.The changes in S100A8/A9,CS-846 and MMP3 levels have predictive value for knee osteoarthritis.
