摘要
目的探讨口腔运动干预在复杂型先天性心脏病患儿术后喂养技能改善中的效果。方法将2018年1-12月我中心收治的复杂型先心病患儿255例作为研究对象,根据纳排及剔除排准,按收治时间分为观察组(119例)和对照组(114例)。对照组给予常规喂养,观察组在此基础上采用口腔运动干预。观察两组患儿的喂养表现(喂养效率、喂养成效、摄入奶量百分率)、喂养进程、喂养不耐受发生率、喂养时生命体征改变等指标。结果两组在喂养表现上,观察组优于对照组,差异有统计学意义(P<0.05)。在完成经口喂养目标奶量50%及完成经口喂养目标奶量80%所用时间比较,观察组优于对照组,差异有统计学意义(P<0.05);两组未发生腹泻和血便,在呕吐、腹胀、胃潴留的比较,观察组发生例数少于对照组,差异有统计学意义(P<0.05);在心率、呼吸、脉氧饱和度改变上的比较,差异无统计学意义(P>0.05)。结论口腔运动干预可改善复杂型先心病患儿经口喂养效率,安全加速经口喂养进程,并可减少复杂型先心病患儿喂养不耐受的发生。
Objective To explore the effect of oral exercise intervention on improving feeding skills in children with complex congenital heart diseases.Methods The children with complex congenital heart diseases treated in our center from January 2018 to December 2018 were divided into experimental group(119 cases)and control group(114 cases)according to random number table.The control group was given routine feeding.The oral exercise intervention method used in the experimental group was an intervention plan,including 12 min oral massage(OS)plus 3 min non-nutritive sucking(NNS).The feeding efficiency,feeding success,milk intake percentage,feeding process,feeding intolerance,and changes in vital signs during feeding were observed between the two groups of children with complex congenital heart diseases.Results There was significant difference in the time taken to complete 50%oral feeding target milk volume and 80%oral feeding target milk volume(P<0.05).There was no diarrhea and bloody stool in the two groups during the study period,there was a significant difference between the two groups in vomiting,abdominal distention and gastric retention(P<0.05).There was no significant difference in heart rate,respiration and pulse oxygen saturation(P>0.05).Conclusion Oral exercise intervention can improve the efficiency of oral feeding,accelerate the process of oral feeding.It's safe,Oral exercise intervention can reduce the incidence of feeding intolerance in children with complex congenital heart disease.
