CEA联合sHLA-G、PG、DKK-1在胃癌早期诊断及淋巴结转移预测中的临床价值 被引量:10

Clinical Value of CEA Combined with sHLA-G,PG and DKK-1in the Early Diagnosis and Lymph Node Metastasis Prediction of Gastric Cancer

摘要 目的评估血清癌胚抗原(carcinoembryonic antigen,CEA)、可溶性人类白细胞抗原G(soluble human leukocyte antigen G,sHLA-G)、胃蛋白酶原(pepsinogen,PG)、人类分泌性蛋白Dickkopf-1(human secreted protein Dickkopf-1,DKK-1)在胃癌早期诊断及淋巴结转移预测中的价值。方法选取2017-03/2019-12月于作者医院确诊的100例早期胃癌患者纳入胃癌组,将同期70例良性病变者(慢性胃炎、胃溃疡、胃出血等)纳入良性病变组,另选择同期健康体检者50例作为对照组,检测并对比各组入选者血清CEA、sHLA-G、胃蛋白酶原比(pepsinogen ratio,PGR)(PGⅠ/PGⅡ)、DKK-1水平及阳性表达率,以病理活检结果为金标准,分析CEA、sHLA-G、PG、DKK-1单一或联合诊断淋巴结转移的价值。结果胃癌组血清CEA、sHLA-G、DKK-1水平均高于良性病变组和对照组,PGR水平低于良性病变组和对照组,组间比较差异有统计学意义(P<0.05)。胃癌组CEA、sHLA-G、PG、DKK-1单一及4项指标联合检测的阳性表达率均比对照组、良性病变组高,差异有统计学意义(P<0.05);CEA、sHLA-G、PG、DKK-1联合诊断淋巴结转移的灵敏度(89.29%)、特异度(86.36%)、准确度(88.00%)均比单一检测高,差异有统计学意义(P<0.05)。结论血清CEA、sHLA-G、PG、DKK-1在胃癌早期诊断中有一定的临床价值,可作为潜在生物标记物,但联合检测预测淋巴结转移的灵敏度、特异度均较高,可为淋巴结转移早期诊断、早期预防提供有利依据。 Objective To evaluate the value of serum carcinoembryonic antigen(CEA),soluble human leukocyte antigen G(sHLA-G),pepsinogen(PG),and human secreted protein Dickkopf-1(DKK-1)in early diagnosis and lymph node metastasis prediction of gastric cancer.Methods A total of 100patients with early gastric cancer diagnosed in the author's hospital from March 2017to December 2019were selected as gastric cancer group,and 70patients with benign lesions(chronic gastritis,gastric ulcer,gastric bleeding,etc.)in the same period were included in the benign lesion group,and 50healthy physical examinees in the same period were selected as the control group.The serum CEA,sHLAG,pepsinogen ratio(PGR)(PGⅠ/PGⅡ),DKK-1levels and positive expression rates were detected and compared in each group.The value of CEA,sHLA-G,PG,DKK-1single and combined diagnosis of lymph node metastasis were analyzed according to the results of pathological biopsy.Results The levels of serum CEA,sHLA-G and DKK-1in gastric cancer group were higher than those in benign lesion group and control group,and the level of PGR were lower than those in benign lesion group and control group(P<0.05).The positive rate of CEA,sHLA-G,PG,DKK-1and combined detection of 4indexes in gastric cancer group were higher than those in control group and benign lesions group(P<0.05).The sensitivity(89.29%),specificity(86.36%)and accuracy(88.00%)of CEA,SHLA-G,PG and DKK-1combined in the diagnosis of lymphatic metastasis were higher than those of single detection(P<0.05).Conclusion Serum CEA,sHLA-G,PG,and DKK-1have certain clinical value in the early diagnosis of gastric cancer and can be used as potential biomarkers.However,the sensitivity and specificity of combined detection for predicting lymph node metastasis are relatively high,which can provide favorable basis of early diagnosis and prevention of lymph node metastasis.
