摘要
目的评价二甲双胍联合炔雌醇环丙孕酮治疗多囊卵巢综合征不孕症的临床效果及对激素水平的改善价值。方法选取2018年12月~2019年12月我院妇科收治的多囊卵巢综合征不孕症患者118例为研究对象。炔雌醇环丙孕酮组59例采用炔雌醇环丙孕酮片进行治疗,二甲双胍+炔雌醇环丙孕酮组59例采用二甲双胍片联合炔雌醇环丙孕酮片进行治疗。评价两组患者成熟卵泡个数、促排卵天数、黄体生成激素、胰岛素、促卵泡成熟激素、睾酮、雌二醇水平值、临床治疗优良率、不良反应发生概率。结果治疗3个月后,二甲双胍+炔雌醇环丙孕酮组成熟卵泡个数、FSH、E2、临床治疗优良率均明显高于炔雌醇环丙孕酮组,促排卵天数明显短于炔雌醇环丙孕酮组,LH、胰岛素、T水平值均明显低于炔雌醇环丙孕酮组(P<0.05),不良反应发生率与炔雌醇环丙孕酮组比较,差异无统计学意义(P>0.05)。结论多囊卵巢综合征不孕症患者采用二甲双胍联合炔雌醇环丙孕酮的临床效果及对激素水平的改善价值更加显著,值得推广应用。
Objective To evaluate the clinical effect of metformin combined with ethinylestradiol and cyproterone acetate in the treatment of infertility with polycystic ovary syndrome and its improvement value on hormone levels.Methods A total of 118 cases of infertility patients with polycystic ovary syndrome admitted from December 2018 to December 2019 in the gynecology department of our hospital were included in the study.The ethinylestradiol and cyproterone acetate group(59 cases)was treated with ethinylestradiol and cyproterone acetate tablets,and the metformin+ethinylestradiol cyproterone group(59 cases)was treated with metformin tablets combined with ethinylestradiol and cyproterone acetate tablets.The number of mature follicles,days of ovulation induction,luteinizing hormone,insulin,follicle stimulating hormone,testosterone,estradiol level,clinical treatment excellent rate,and probability of adverse reactions between two groups were evaluated.Results After 3 months of treatment,the number of mature follicles,FSH and E2 in the metformin+ethinylestradiol cyproterone group were significantly higher than those in the ethinylestradiol and cyproterone acetate group,the days of ovulation induction in the metformin+ethinylestradiol cyproterone group were significantly shorter than those of the ethinylestradiol and cyproterone acetate group,the levels of LH,insulin and T in the metformin+ethinylestradiol cyproterone group were significantly lower than those in the ethinylestradiol and cyproterone acetate group(P<0.05).There were no significant differences in the incidence of adverse reactions between the metformin+ethinylestradiol cyproterone group and the ethinylestradiol and cyproterone acetate group(P>0.05).Conclusion The metformin combined with ethinylestradiol and cyproterone acetate in patients with infertility with polycystic ovary syndrome has more significant clinical effect and improvement value in hormone levels,and it is worthy of popularization and application.
