摘要
目的分析阴茎闭合性外伤的MRI表现,探讨高场MRI对阴茎海绵体外伤的诊断价值。方法12例经证实为阴茎海绵体闭合性外伤患者,患者年龄21~50岁,平均年龄35岁。所有病例均行3.0 T MR扫描及超声检查,结合临床资料回顾性分析磁共振表现。结果12例患者均为阴茎勃起状态下暴力致伤,其中暴力性生活9例,手淫2例,撞伤1例。12例阴茎海绵体闭合性损伤中:(1)阴茎海绵体损伤9例,其中2例阴茎海绵体损伤伴海绵体血肿,1例阴茎海绵体损伤伴Buck’s筋膜间少量积血;单侧阴茎海绵体损伤并白膜撕裂7例,双侧1例。MRI表现为阴茎皮下筋膜肿胀,阴茎海绵体局部白膜T1WI/T2WI上正常低信号中断,局部白膜缺损。血肿较大可从破口处疝出。(2)单纯阴茎海绵体血肿2例,表现为阴茎海绵体内斑片T1WI和T2WI低信号,呈慢性血肿改变,增强扫描血肿周围海绵体组织斑片强化。(3)阴茎海绵体白膜与Buck’s筋膜间血肿1例,阴茎局部皮下肿胀,白膜与Buck’s筋膜间可见T1WI和T2WI高信号的小血肿。MR结合小线圈高分辨率成像对12例患者均作出正确诊断;超声检出9例,有2例白膜轻度撕裂和1例阴茎白膜与Buck’s筋膜间小血肿超声未检出,1例白膜撕裂误诊。结论采用3.0 T MR小线圈成像能更好地诊断阴茎闭合性损伤,特别是对微小病变的显示。
Objective To assess the diagnostic value of MRI in closed penile trauma.Methods Twelve men with closed penile trauma(age range:21-50 years,mean:35 years)underwent ultrasonography(US)and 3 T MRI with small coil.The MRI was analyzed retrospectively based on clinical data.Results All patients sustained erect penile injury during violent sexual intercourse(9),masturbation(2)or collision(1)including corpus cavernosum rupture(9)with cavernosal hematoma(2)or small hematoma between the tunica albuginea and Buck’s fascia(1).Compared to MRI that depicted all injury,US was positive in 9/12 patients.There was rupture of the unilateral(7)or bilateral(1)corpora cavernosa.MRI showed swelling of the penile fascia with interruption of the T1-and T2-hypointense tunica albuginea.The cavernosal hematomas showed low T1 and T2 signal intensities with rim enhancement.One hematoma between the tunica albuginea and Buck’s fascia with focal subcutaneous swelling of the penis was T1-and T2-hyperintense on MRI and not detected on US.Small tunica albuginea tear was also not visible on US in 2 patients.Conclusion 3 T MRI with small coil is superior to US for detecting closed penile trauma.
