摘要
目的探讨血嗜酸性粒细胞水平与氢化泼尼松对慢性阻塞性肺疾病急性加重期(AECOPD)患者预后差异的相关性。方法回顾2018年7月~2018年12月在呼吸内科收住的AECOPD患者,以治疗时首次获得的血嗜酸性粒细胞百分比分组:Group H(嗜酸性粒细胞百分比≥5%,n=15)、Group M(2%≤嗜酸性粒细胞百分比<5%,n=54)、Group L(嗜酸性粒细胞百分比<2%,n=142)。收集归纳患者基本信息、血常规、C反应蛋白(CRP)、白介素6(IL-6)、肝肾功能、D-二聚体(D-dimer)、住院天数、氢化泼尼松总量以及住院治疗4 d后患者预后差异并统计分析。结果Group M、Group H的CRP、D-dimer较Group L明显降低(P<0.05),住院治疗显效率+有效率之和显著提高(P<0.05);同时,住院天数明显缩短(P<0.05),动脉血气改善明显(P<0.05)。经多因素线性回归分析,血嗜酸性粒细胞水平与CRP、D-dimer、DLCO/VA具有独立的相关性,其中,与CRP、D-dimer呈负相关,与DLCO/VA呈正相关。结论全身用氢化泼尼松对于外周血嗜酸性粒细胞水平增高的AECOPD患者预后更好。
Objective To explore the correlation between blood eosinophil levels and prednisolone on prognosis of patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods The AECOPD patients admitted in the Department of Respiratory Medicine from July 2018 to December 2018 in our hospital were retrospectively grouped by the percentage of eosinophil first obtained during treatment:Group H(eosinophil percentage≥5%,n=15),Group M(2%≤eosinophil percentage<5%,n=54),Group L(eosinophil percentage<2%,n=142).General patient information,blood routine,C-reactive protein(CRP),interleukin 6(IL-6),liver and kidney function,D-dimer,length of hospital stay,total prednisolone,and Differences in prognosis of patients after 4 days of hospitalization were analyzed statistically.Results The CRP and D-dimer of Group M and Group H were significantly lower than those of Group L(P<0.05),and the sum of hospitalization efficiency and effective rate was significantly improved(P<0.05);meanwhile,the number of hospitalization days was significantly shortened(P<0.05),the arterial blood gas improved significantly(P<0.05).Multivariate linear regression analysis revealed that CRP,D-dimer and the diffusing capacity of carbon monoxide(DLCO)to alveolar volume(VA)ratio(DLCO/VA)was independently.Meanwhile,CRP and D-dimer were negatively correlated with blood eosinophil count,while DLCO/VA was positively correlated.Conclusion Systemic prednisone have a better prognosis on patients with AECOPD with elevated eosinophil levels in peripheral blood.
