摘要
目的比较不同表面结构、不同失牙原因对下颌前牙区即刻种植效果的影响,从而指导临床对下前牙区不同种植系统,不同表面结构进行选择,获得良好的初期稳定性及远期疗效。方法收集科室101颗下颌前牙区即刻种植病例,具有三种不同的表面结构,其中光滑圆柱状(BLB系统)11颗,螺旋柱状(ITI系统)39颗,双螺旋根形(OSSTEM系统)51颗;因牙周病失牙61颗,因外伤失牙40颗,对其失败率及是否需植骨进行比较分析。结果共植入种植体101颗,成功92颗,失败9颗,植骨病例81颗,失败病例均为植骨病例。失牙原因其中牙周病61颗,失败9颗;外伤40颗,失败0颗。BLB失败率、植骨百分比与ITI、OSSTEM对比,差异均具有统计学意义,P<0.05。结论不同表面结构、不同失牙原因对下颌前牙区即刻种植的成功是有影响的,对骨量的需求也是不同的,下前牙即刻种植的难点主要集中在骨量的问题,而选用多螺纹带有自攻性的种植体对于种植获得良好的初期稳定性有很大帮助,从而使种植体获得良好的远期疗效。
Objective To compare the effects of different surface structures and different causes of tooth loss on the immediate implant effect in mandibular anterior region,so as to guide the clinical selection of different implant systems and surface structures in the lower anterior region,so as to obtain good initial stability and longterm effect.Methods 101 cases of immediate implant in mandibular anterior region were collected.There were 11 smooth cylindrical(BLB system),39 spiral columnar(ITI system)and 51 double spiral root(OSSTEM system).61 teeth were lost due to periodontal disease and 40 teeth were lost due to trauma.Results A total of 101 implants were implanted,of which 92 were successful and 9 were failed.81 cases were bone graft cases.The causes of tooth loss included periodontal disease in 61 cases,failure in 9 cases,trauma in 40 cases,failure in 0.Compared with ITI and OSSTEM,the failure rate and bone graft percentage of BLB were P<0.05.Conclusion Different surface structures and different causes of tooth loss have an impact on the success of immediate implant placement in the mandibular anterior region,and the demand for bone mass is also different.
