摘要
膜性肾病病性多属邪实正虚、虚实夹杂,病机关键是以湿热瘀阻、气化不利为主,继而影响三焦及肺、脾、肾等脏腑的生理功能。湿热之邪是本病重要的致病因素。临床中,应以“调补分化”法治疗本病,即调畅气机、宣通三焦;补益虚损、健运三焦;分消湿热、疏利三焦;化瘀活血、凉血清热。临证时,根据疾病的不同阶段辨明湿热偏颇轻重,三焦同调,肺、脾、肾兼治,注重察舌验脉是否相契合,谨守病机,随症化裁,平和缓图,切不可概以虚证论之而孟浪进补。
The pathogenesis of membranous nephropathy(MN)is mostly the combination of excess of pathogenic qi and deficiency vital qi,intermingled deficiency and excess.The key pathogenesis is stagnant dampness-heat and disturbance of qi transformation,which then affects the physiological functions of Sanjiao,lung,spleen and kidney.The dampness-heat pathogen is an important pathogenic factor of this disease.In clinical practice,the method of"regulating,reinforcing and differentiating"should be used to treat this disease,that is,regulating the qi movement and circulating Sanjiao;reinforcing deficiency,strengthening and promoting the movement of Sanjiao;subdividing damp-heat,relieving Sanjiao;removing blood stasis and promoting blood circulation,cooling blood and clearing heat.In clinical practice,it is necessary to distinguish the severity of dampness-heat according to the different stages of the disease.Regulate Sanjia simultaneously,and treat lung,spleen and kidney simultaneously.Pay attention to checking whether the tongue and pulse are consistent,and pay attention to the pathogenesis,adapting the prescription to the diseas.Be gentle and slow,and it must not be toniced quickly because of deficiency.
