首次冻融移植周期中阿托西班应用时机对妊娠结局的影响 被引量:5

Effect of timing of atosiban application on pregnancy outcome in the first frozen-thawed transplantation cycle

摘要 目的探讨首次冻融移植周期中阿托西班应用时机对妊娠结局的影响。方法选择2017年7月至2019年6月首次行冻融移植患者,利用倾向性评分匹配法共纳入1682例患者的临床资料。根据冻融移植前是否使用阿托西班分为阿托西班组和空白对照组,阿托西班组841例患者于移植前30 min使用阿托西班6.75 mg/0.9 ml静脉推注,对照组841例未使用药物。分析比较两组移植不同时期胚胎之间着床率、临床妊娠率、异位妊娠率、早期流产率及多胎率。结果首次移植卵裂期胚胎,阿托西班组的临床妊娠率显著高于对照组(47.01%vs.37.56%)(P<0.05),着床率、早期流产率和多胎率差异均无统计学意义(P>0.05);而首次移植囊胚期胚胎的阿托西班组与对照组相比,各临床结局指标均无统计学差异(P>0.05)。结论阿托西班对首次移植不同时期胚胎的临床结局影响是不同的,移植前小剂量阿托西班静推可提高首次卵裂期胚胎移植的临床妊娠率。 Objective:To explore the effect of timing of atosiban application on pregnancy outcome in the first frozen-thawed transfer cycle.Methods:The patients who underwent the first frozen-thawed embryo transfer from July 2017 to June 2019 were selected.The clinical data of 1682 patient were included using the propensity score matching method.According to whether atosiban was used before transplantation,the patients were divided into atosiban group(n=841)and the control group(n=841).The patients in the atosiban group received an intravenous bolus of atosiban 6.75 mg/0.9 ml,30 minutes before transplantation,and the patients in the control group did not use any drug.The implantation rate,clinical pregnancy rate,ectopic pregnancy rate,early miscarriage rate and multiple pregnancies of embryos in different stages were analyzed and compared between the two groups.Results:The clinical pregnancy rate in cleavage-stage embryos for the first transplant in the atosiban group was significantly higher than that of the control group(47.01%vs.37.56%)(P<0.05).There was no significant difference in early abortion rate and multiple pregnancies(P>0.05).When the embryos at blastocyst stage were transferred,no significant difference was found in clinical outcomes between the atosiban group and the control group(P>0.05).Conclusions:Atosiban has different effects on the clinical outcomes of embryos at different stages of the first transplantation.A small dose of atosiban used before transplantation can increase the clinical pregnancy rate of the cleavage-stage embryos of the first transfer cycle.
