摘要
本文通过整理卢苏教授对子宫内膜息肉的病因病机认识、治疗特色及验案分析,总结其辨治子宫内膜息肉的经验。认为子宫内膜息肉是虚实夹杂之证,其病机之本在肾虚,病因责之血瘀。肾虚与血瘀相互影响,肾虚可导致瘀血形成,并不利于使停滞胞宫的瘀血排泄,瘀血留滞胞宫形成癥积;七情内伤、金刃创伤导致血瘀,不利于气血津液的运行疏布,难养先天。治疗上,主张补肾调周法结合活血化瘀,分经期、经后期、经间期、经前期4期论治,重视经前期补肾助阳,顺应经期活血化瘀的治疗,术后调理提倡脾肾同补。
This article discusses professor LU Su's experience in the diagnosis and treatment of endometrial polyps by sorting out her understanding of the etiology and pathogenesis,treatment characteristics and case analysis.Lu Su believed that endometrial polyps was intermingled deficiency and excess,the pathogenesis of which was kidney deficiency and blood stasis.Kidney deficiency and blood stasis affect each other:kidney deficiency can lead to blood stasis,and is not conducive to blood stasis in the uterus excretion,blood stasis stagnation of the formation of the disease;Internal injury of moods and surgical trauma lead to blood stasis,which is not conducive to the distribution of Qi,blood and body fluid,and difficult to nourish kidney.In terms of treatment,Lu Su advocates invigorating kidney and regulating menstruation cycles combined with promoting blood circulation and removing blood stasis,which can be divided into four stages:menstrual period,late menstrual period,intermenstrual period and early menstrual period.Attention should be paid to tonifying the kidney and assisting Yang in the early menstrual period,and the treatment of promoting blood circulation and removing blood stasis in the menstrual period,promoting the simultaneous repair of spleen and kidney after surgery.
