摘要
目的:探讨支气管源性囊肿的临床表现、病理特征及诊断、治疗方法,提高支气管源性囊肿的诊治水平。方法:回顾性分析我院59例支气管源性囊肿的临床资料。结果:根据X线片、胸部CT检查等结果,本组病例中纵隔型44例、肺内型14例、异位型2例(其中1例表现为前纵隔及右肺下叶多发病变),所有病例均行手术治疗并病理证实,其中术前误诊25例(误诊率41.7%),手术方式包括囊肿局部切除,胸腺及周围脂肪切除,肺叶切除,肺楔形切除及肺段切除,术后均治愈出院。结论:支气管源性囊肿发病部位、临床及影像学表现缺乏特异性,术前诊断较为困难,应通过综合多种影像检查来诊断,手术切除是最佳治疗方案,其确诊仍需手术切除后病理证实。
Objective:To investigate the clinical manifestations,pathological features,diagnosis and treatment of bronchogenic cysts,for improving level of the diagnosis and treatment of bronchogenic cysts.Methods:The clinical data of 59 bronchogenic cysts in our hospital were retrospectively analyzed.Results:According to the results of X-ray films and chest CT examinations,44 cases of mediastinal type,14 cases of intrapulmonary type and 2 cases of ectopic type(one of which showed multiple lesions of anterior mediastinum and right lower lobe)were included in this group.All cases were surgically treated and pathologically confirmed.Among them,25 cases were misdiagnosed before operation(missing rate was 41.7%).The surgical methods included local cystectomy,thymus and peripheral fat resection,lobectomy,pulmonary wedge resection and segmen-tal resection.All patients were cured and discharged from hospital.The location,clinical and imaging findings of bronchogenic cysts are lack of specificity.Preoperative diagnosis is difficult.It should be diagnosed by comprehensive imaging examination.Surgical resection is the best treatment plan.The diagnosis still needs pathological confirmation after surgical resection.
