某三级儿童医院住院患者医院感染现患率调查分析 被引量:2

Investigation and analysis of the prevalence of nosocomial infections among hospitalized patients in 2019in a grade 3 Children's Hospital

摘要 目的了解医院感染发病率、感染部位、抗菌药物使用与细菌耐药情况等,提高医院感染监测质量,为医院感染防控提供依据。方法选取2019年某天对医院所有住院患者进行医院感染现患率调查,将调查名单及调查表下发各临床科室,并对临床医生进行现场培训并督促填写完善,收集所有资料进行汇集分析。结果本次共调查患者359例,实查率100%(359/359),医院感染13例,共14例次,医院感染现患率3.62%,例次现患率3.9%。现患率最高的科室为血液科(12.5%)、新生儿科(7.32%)、神经内科(6.45%);医院感染部位居前三位的是血流感染(6/14,42.9%)、下呼吸道感染(4/14,28.6%)、上呼吸道感染(2/14,14.3%);抗菌药物使用率为67.7%(243/359),使用抗菌药物的243例中送检细菌培养162例,细菌培养送检率为66.7%(162/243),其中,抗菌药物使用前细菌培养送检率为60.5%(98/162)。结论医院感染现患率调查能较好反应本院医院感染情况,加强对院内感染高风险科室血液科、新生儿科及高风险部位血液感染、呼吸道感染的医院感染管理工作,可能会降低医院感染的发生。 Objective To investigate the incidence and composition of nosocomial infection sites, antibacterial drug use and bacterial drug resistance of pathogens in hospitalized children in a children’s hospital, improve the quality of nosocomial infection monitoring, and provide scientific basis for prevention and control of nosocomial infections.Methods On a chosen date, we carried out a prevalence rate survey among all the hospitalized in the hospital. The survey list and questionnaire will be sent to the clinical departments, and clinicians were trained on-site and urged to complete the questionnaire, all data were collect for analysis.Results We investigated 359 cases of hospitalized patients on the chosen date, and the actual investigation rate was 100%. We found 13 cases of nosocomial infections and totally infected for 14 times, and the prevalence of current nosocomial infections and cases current nosocomial infections were 3.62% and 3.9%, respectively. The top 3 departments of nosocomial infection were pediatric hematology(12.5%), neonatal intensive care unit(7.32%) and pediatric neurology(6.45%). The main infection sites were blood tract(42.9%), lower respiratory tract(28.6%), and upper respiratory tract(14.3%). The utilization rate of antibiotics in the hospitalized patients is 67.7%(243/359). 162 cases conducted microbiological examination of the 243 cases utilizing antibiotics. The rate of microbiological examination was 66.7%(162/243). The rate of microbiological examination before utilizing antibiotics was 60.5%(98/162).Conclusions The survey accurately reflects the status of nosocomial infections. To enhance the management of department of hematology and neonatal intensive care unit, and focus on the infection of blood tract and respiratory tract may reduce the incidence of nosocomial infections.
