视觉反馈训练联合体外冲击波治疗对脑卒中患者平衡功能的影响 被引量:4

Effects of Visual Feedback Training Combined with Extracorporeal Shock Wave Therapy on Balance Function in Stroke Patients

摘要 目的:观察视觉反馈训练联合体外冲击波治疗对脑卒中患者平衡功能的影响。方法:选取符合入选标准的脑卒中患者81例,按随机数字表法分为对照组、视觉反馈组、联合治疗组各27例,均接受常规康复治疗,每天1次,连续进行2周;视觉反馈组在常规康复治疗同时加入视觉反馈训练,每天1次,连续进行2周;联合治疗组在视觉反馈组基础上加入体外冲击波治疗,每周3次(隔天治疗),连续进行2周。于治疗前、治疗2周后对三组患者分别采用改良Ashworth量表(MAS)、简化Fugl-Meyer评定法(FMA)下肢部分、Berg平衡量表(BBS)及BioFlex-FP姿势控制评估与训练系统自带的站立稳定性测试软件进行评定。结果:治疗前,三组患者各项得分差异均无统计学差异(P> 0.05)。治疗2周后,发现对照组、视觉反馈组MAS得分差异无统计学意义(P> 0.05),组间比较发现联合治疗组MAS得分相比对照组、视觉反馈组改善幅度较显著且差异有统计学意义(P <0.05);治疗2周后,组内比较发现三组患者除MAS评分外其余各项数值均较治疗前显著改善(P <0.05),且联合治疗组疗效更为显著(P <0.05)。结论:视觉反馈训练与体外冲击波治疗在脑卒中患者的平衡功训练中有协同作用,联合治疗能进一步改善脑卒中患者的平衡功能。 Objective: To observe the effect of visual feedback training combined with extracorporeal shock wave therapy on the balance function of stroke patients. Methods: 81 patients with shock who met the inclusion criteria were selected and divided into control group(n=27), visual feedback group(n=27) and combined treatment group(n=27)according to the random number table method. After enrollment, all patients received conventional rehabilitation treatment once a day for 2 weeks. In the visual feedback group, visual feedback training was added at the same time of routine rehabilitation, once a day for two weeks. In the combined treatment group, on the basis of visual feedback group, extracorporeal shockwave therapy was added at the same time, three times a week(alternate day treatment)for two consecutive weeks. The modified Ashworth scale(MAS) and simplified Fugl-Meyer Assessment(FMA)lower limb Berg balance scale(BBS)were used for the three groups of patients before and after treatment, and the BioFlex-FP posture control evaluation and the standing stability test software built into the training system were used for the evaluation. Results: Before treatment, there was no significant difference in scores among the three groups(P > 0.05). After 2 weeks of treatment, it was found that there was no significant difference in MAS scores between the control group and the visual feedback group(P > 0.05), and the inter-group comparison showed that the improvement of MAS scores in the combined treatment group was significant and the difference was statistically significant(P < 0.05). After 2 weeks of treatment, intra-group comparison showed that all the values in the 3 groups were significantly improved except for MAS score(P < 0.05). The efficacy of the combined treatment group was more significant(P < 0.05). Conclusion: Visual feedback training and extracorporeal shock wave therapy have a synergistic effect on the balance work training of stroke patients, and combined therapy can further improve the balance function of stroke patients.
