摘要
目的探讨血浆置换法对肿瘤患者乳糜血全血细胞分析的纠正作用。方法选取2019年1月至2019年12月接收的经全血细胞分析后明确的乳糜血标本28例。先加入2 mL稀释液,依据乳糜血程度的不同,进行1~3次的血浆置换。重新进行全血细胞分析测定。比较置换前和置换后的白细胞(white blood cell,WBC)、红细胞(red blood cell,RBC)、血红蛋白(hemoglobin,HGB)、红细胞比容(hematocrit,HCT)、平均红细胞体积(mean corpuscular volume,MCV)、平均红细胞血红蛋白量(mean corpuscular hemoglobin,MCH)、平均红细胞血红蛋白浓度(mean corpuscular hemoglobin concentration,MCHC)、血小板(platelet,PLT)等指标的结果。结果乳糜血标本因报警信息,全血细胞计数结果异常等情况而被发现。肿瘤患者乳糜血中结直肠癌所占比例最高。乳糜血标本经血浆置换后MCHC恢复到正常和(或)稳定的状态或与历史结果相符,表明已纠正乳糜血影响。血浆置换前后,全血细胞计数结果中的Hb、MCH、MCHC和PLT四项指标的差异具有统计学意义(P<0.05);而RBC、HCT、MCV和WBC的差异无统计学意义(P>0.05)。结论乳糜血干扰的主要参数指标为Hb、MCH、MCHC和PLT,血浆置换法可纠正这些被影响指标的结果,具有推广价值。
Objective To investigate the corrective effect of plasma replacement method on the whole blood cell analysis of chylous blood in cancer patients.Methods Twenty-eight chylous blood samples confirmed by the whole blood cell analysis from January,2019 to December,2019 were selected for the study.These samples were firstly diluted with 2mL diluent,and then plasma replacement was carried out 1-3 times according to the chylous degree.The whole blood cell analysis of the samples after the replacement was measured.In particular,the results of white blood cell(WBC),red blood cell(RBC),hemoglobin(Hb),mean corpuscular hemoglobin(MCH),platelet(PLT),mean corpuscular volume(MCV),hematocrit(HCT),and mean corpuscular hemoglobin concentration(MCHC)were compared.Results Chylous blood samples were found according to alarm messages,and abnormal whole blood cell count results,etc.Patients of colorectal cancer had the highest proportion of chylous blood in tumor patients.MCHC returned to normal and(or)stable state after plasma exchange or consistent with historical results,indicating the effects of chyle blood had been corrected.Comparing the results of before and after plasma exchange,the differences in Hb,MCH,MCHC,and PLT indicators were statistically significant(P<0.05),while the differences in RBC,HCT,MCV,and WBC were not statistically significant(P>0.05).Conclusion In chylous blood samples,the measurements of Hb,MCH,MCHC and PLT are significantly affected.The deviation of the measurement in chylous whole blood cell count can be corrected by plasma replacement method,which has a popularization value.
