某院1065例尿路感染病原菌分布及耐药性分析 被引量:3

摘要 目的了解该院尿路感染病原菌的分布与耐药情况,为临床治疗提供参考。方法收集该院2018年1月至2019年12月所有尿培养阳性菌株,对1065株病原菌进行菌种鉴定和药敏试验。结果共有11253份中段尿培养标本送检,在剔除相同患者的重复菌株后分离出有效菌株1065株。以革兰阴性菌(777株)为主,占73.0%;革兰阳性菌(226株)占21.2%;真菌(62株)占5.8%。排名前5位的病原菌分别是大肠埃希菌(45.7%)、粪肠球菌(8.7%)、肺炎克雷伯菌(8.6%)、屎肠球菌(6.0%)、白色念珠菌(3.6%)。肠杆菌科细菌中,产超广谱β-内酰胺酶(ESBLs)的大肠埃希菌225株(47.6%)、产ESBLs的肺炎克雷伯菌30株(32.6%)。主要革兰阳性菌中,未检出对万古霉素、替考拉宁耐药的肠球菌。结论不同地区尿路感染病原菌和耐药特点也不同,应及时总结、分析当地病原菌分布特点及耐药性,为临床合理应用抗菌药物提供参考依据。 Objective To investigate the distribution and drug resistance of pathogenic bacteria of urinary tract infection in this area,and to provide reference for clinical treatment.Methods Collection of positive urines pecimens from the hospitalized patients from January 2018 to December 2019,analysis of the identification of 1065 strains of pathogenic bacteria and the result of drug sensitivity test were performed.Results A total of 11253 midcourse urine culture samples were submitted for examination,and 1065 effective strains were isolated after the repeated strains of the same patients were removed.The gram-negative bacteria(777 strains)were dominant,accounted for 73.0%,gram positive bacteria(226 strains)accounted for 21.2%;fungi(62 strains)accounted for 5.8%.The top five pathogens were escherichia coli(45.7%),enterococcus faecalis(8.7%),klebsiella pneumoniae(8.6%),enterococcus faecalis(6.0%)and candida albicans(3.6%).Among the enterobacteriaceae bacteria,225 strains(47.6%)of large intestine producing-lactamase(ESBLs)and 30 strains(32.6%)of klebsiella pneumoniae producing ESBLs were produced.Among the main gram-positive bacteria,there were no enterococcus resistant to vancomycin and teicolanin.Conclusion The characteristics of pathogenic bacteria and drug resistance of urinary tract infection are also different in different areas,should be made in time to provide reference for the rational application of antibiotics in clinic.
