摘要
目的:探讨“齐鲁模式”移动ICU院间转运患者的变化趋势,为安全转运患者寻找切实可行的措施提供依据。方法:选取2014年8月1日~2019年7月31日“齐鲁模式”移动ICU院间转运的358例患者,回顾性收集其病历和护理记录单,分析并比较5年间患者临床特征的变化情况。结果:转运相关指标:5年间院间转运的距离、转运时间段、患者的来源医院、转入科室的比较差异均无统计学意义(P>0.05)。患者相关指标:5年间转运患者的年龄、性别、住院时间、疾病分类、转入ICU患者的急性生理与慢性健康评分(APACHEⅡ)、序贯器官衰竭评分(SOFA)的比较差异均无统计学意义(P>0.05);5年间患者应用人工气道比例、机械通气比例、预后、转入ICU患者的首次Braden评分的比较差异均有统计学意义(P<0.05)。转运不良事件:共12例,其中与呼吸机不匹配有关的为5例,与病情变化有关的为6例,气管插管脱出1例。结论:5年间“齐鲁模式”移动ICU院间转运的患者,其临床特征有一定变化趋势,院间转运应及时、合理,以保障患者安全。
Objective:To explore the change trend of Qilu model mobile ICU patients in inter-hospital transport,so as to provide basis for finding feasible measures for safe transport of patients.Methods:A total of 358 patients in Qilu model mobile ICU were selected from August 1,2014 to July 31,2019.Their medical records and nursing records were collected retrospectively,and the changes in clinical characteristics of patients were analyzed and compared over 5 years.Results:There were no statistically significant differences in the transport distance between hospitals,transport time period,the source hospital of patients,and the departments transferred within 5 years(P>0.05).The indicators related to patient:there were no statistically significant differences in age,gender,length of hospital stay,disease classification,APACHEⅡscore and SOFA score of patients transferred to ICU within 5 years(P>0.05).There were statistically significant differences in the proportion of patients using artificial airway,proportion of mechanical ventilation,prognosis,and the first Braden score of patients transferred to ICU within 5 years(P<0.05).There were 12 adverse events of transport in total.Among them,5 cases were related to ventilator mismatch,6 cases were related to condition change,and 1 case was endotracheal intubation dislocation.Conclusion:The clinical characteristics of the patients transferred by Qilu model mobile ICU in the past 5 years showed a certain trend of changes,and the transfer between hospitals should be timely and reasonable to ensure patient safety.
