新型宽体探测器CT对先天性心脏病小儿心脏成像中冠状动脉可视性评估 被引量:7

摘要 目的:研究新型宽体探测器CT在先天性心脏病(CHD)患儿心脏CT成像中冠状动脉的可视性及成像质量。方法:连续纳入172例CHD患儿作为研究对象,按年龄分为4组,组1(年龄≤1岁,97例)、组2(1岁<年龄≤5岁,48例)、组3(5岁<年龄≤10岁,18例)、组4(年龄>10岁,9例)。由两名放射科医师利用4分法对冠状动脉各节段(共11个节段)的显示质量进行主观评价,另一名放射科医师记录升主动脉根部CT值、噪声值、信噪比(SNR)及对比噪声比(CNR)评价客观图像质量。分析评价CHD患儿冠状动脉的可视性并对4组的客观图像质量进行比较。结果:本组CHD患儿冠状动脉异常发生率12.2%,冠状动脉总可视率87.1%,开口、近段、中段及远段可视率依次下降,分别为97.1%(331/341)、90.5%(617/682)、76.6%(262/342)及63.3%(324/512)。冠状动脉11个节段中左主干开口可视率最高98.8%,回旋支远段可视率最低44.2%。不同年龄组间的图像质量与年龄无相关性(P>0.05)。结论:新型宽体探测器CT对CHD患儿行心脏CT扫描成像中对冠状动脉的显示效果佳,可以满足对冠状动脉异常的诊断。 Objective:To investigate the visibility and image quality of coronary artery in children with congenital heart disease(CHD)by wide-detector revolution CT.Methods:Tthere were continuous 172 children with CHD.They were divided into four groups according to age:group 1(no more than 1 year,97 cases),group 2(1 to 5 years,48 cases),group 3(5 to 10 years,18 cases),and group 4(more than 10 years,9 cases).The subjective image quality of coronary artery(totally 11 segments)was evaluated with a four-point scale by two radiologists.Analysis and evaluation of coronary artery visibility in CHD Children were performed.CT value,noise value,signal to noise ratio(SNR)and contrast to noise ratio(CNR)were analyzed to investigate the correlation between image quality and age.Results:The incidence of abnormal coronary artery was 12.2%and the total visibility rate of coronary artery was 87.1%.The visibility of origin,proximal,middle and distal segments were 97.1%(331/341),90.5%(617/682),76.6%(262/342)and 63.3%(324/512),respectively.In 11 segments of the coronary artery,the visibility of left coronary origin was the highest(98.8%),and the visibility of the distal segment of left circumflex artery was the lowest(44.2%).No correlation between image quality and age in four groups(P>0.05).Conclusions:The wide-detector revolution CT has a good capability in displaying coronary artery in children with CHD,and it can provide effective CT imaging for diagnosis of coronary artery anomalies.
