摘要
目的探究影响内镜下黏膜剥离术治疗早期胃癌患者生存的因素。方法回顾性分析90例接受内镜下黏膜剥离手术治疗胃癌患者的相关治疗以及3年随访资料。患者生存率采用Kaplan-Meier计算,单因素与多因素分析分别使用Log-rank分析和Cox回归分析。结果患者接受随访时间为0.9~36个月,中位时间为22.7个月,随访患者1年生存率为100.00%(90/90),3年生存率为91.11%(82/90)。单因素分析显示分化程度、肿瘤大小、Borrmannn分型、肝转移、浸润情况、手术效果、术后并发症、术后辅助化疗为患者3年生存率的影响因素(P<0.05),而性别、年龄、CA19-9水平、病灶中心位置、CEA水平对患者3年生存率无显著影响(P>0.05);多因素Cox回归分析结果显示Borrmannn分型、术后并发症、术后辅助化疗不是患者3年生存率的独立影响因素(P>0.05),分化程度、肿瘤大小、肝转移、浸润情况、手术效果为影响患者3年生存率的独立因素(P<0.05)。结论分化程度、肿瘤大小、肝转移、浸润情况、手术效果为导致早期胃癌患者内镜黏膜下剥离术术后生存率的影响因素。
Objective To investigate the factors influencing the survival of patients with early gastric cancer treated with endoscopic submueosal dissection(ESD).Methods The related treatment data and 3-year follow-up data of 90 patients with gastric cancer treated with ESD were analyzed retrospectively.The survival rates of patients were calculated by Kaplan-Meier.Univariate and multivariate analysis were performed by Log-rank analysis and Cox regression analysis,respectively.Results The duration of follow-up,median time,1-year survival rate and 3-year survival rate were 0.9 to 36 months,22.7 months,100.00%(90/90)and 91.11%(82/90),respectively.Univariate analysis showed that differentiation degree,tumor size,Borrmannn classification,liver metastasis,infiltration,surgical effect,postoperative complications and postoperative adjuvant chemotherapy were factors influencing the 3-year survival rate of patients(P<0.05).Gender,age,CA19-9 level,center position of lesions and CEA level had no obvious effect on the 3-year survival rate(P>0.05).The results of multivariate Cox regression analysis showed Borrmannn classification,postoperative complications and postoperative adjuvant chemotherapy were not independent influencing factors of the 3-year survival rate of patients(P>0.05).Differentiation degree,tumor size,liver metastasis,infiltration,and surgical effect were independent factors influencing the 3-year survival rate of patients(P<0.05).Conclusion Differentiation degree,tumor size,liver metastasis,infiltration and surgical effect are influencing factors of the survival rate of patients with early gastric cancer after ESD.
