摘要
目的 探讨改良截石位及低分子肝素钠在预防妇科恶性肿瘤术后深静脉血栓形成方面的应用价值.方法 选取广东省阳江市人民医院2018年2月~2019年7月收治的88例妇科恶性肿瘤患者为研究对象,采用随机双盲方法分为对照组与观察组,对照组44例:常规手术体位与普通肝素钠,观察组44例:改良截石位及低分子肝素钠,比较两组患者术后1周下肢深静脉形成率、凝血三项指标以及术后并发症发生率.结果 观察组术后1周深静脉血栓形成率小于对照组(P<0.05);观察组术后凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(FIB)等指标与对照组比较,差异有统计学意义(P<0.05);观察组术后并发症发生率小于对照组,差异有统计学意义(P<0.05).结论 妇科恶性肿瘤采取改良截石位以及使用低分子肝素钠,可预防术后深静脉血栓形成,改善凝血指标,降低术后并发症发生,值得推广与应用.
Objective To explore the application value of modified lithotomy position and low molecular heparin sodium in preventing deep vein thrombosis(DVT)after gynecologic malignant tumor surgery.Methods The subjects of the study were 88 patients with gynecologic malignant tumor admitted to People's Hospital of Yangjiang,Guangdong Province from February 2018 to July 2019.The patients were divided into the control group and the observation group by a randomized double-blind method.The control group consisted of 44 patients treated with conventional surgical position and common heparin sodium,while the observation group consisted of 44 patients treated with modified lithotomy position and low molecular heparin sodium.The formation rate of DVT of lower limbs,three coagulation indexes and postoperative complications were compared between the two groups one week after operation.Results The formation rate of DVT in the observation group was lower than that in the control group one week after operation,with P<0.05.The indexes of prothrombin time(PT),activated partial thromboplastin time(APTT)and fibrinogen(FIB)after operation in the observation group were different from those in the control group,with P<0.05.The incidence of postoperative complications in the observation group was lower than that in the control group,with statistical significance and P<0.05.Conclusion Modified lithotomy position and low molecular heparin sodium can prevent postoperative DVT,improve coagulation indexes and reduce postoperative complications for gynecologic malignant tumors,which is worthy of promotion and application.
