摘要
目的观察双环醇联合更昔洛韦及熊去氧胆酸治疗婴儿巨细胞病毒性肝炎的临床效果。方法将2016年9月至2018年8月该院住院并确诊为巨细胞病毒(CMV)性肝炎的小婴儿120例随机分为治疗组与对照组,各60例。两组均给予更昔洛韦针5 mg/(kg·次),2次/d,2周,口服熊去氧胆酸5 mg/(kg·次),2次/d;治疗组再加双环醇片1 mg/(kg·次),2次/d。结果治疗组治疗前谷丙转氨酶(t=2.000)、碱性磷酸酶(t=12.321)、血清总胆红素(t=2.321)、血清直接胆红素(t=5.543)及谷氨酰转肽酶(t=15.432)高于治疗后,差异有统计学意义(P<0.05);对照组治疗前谷丙转氨酶(t=1.913)、碱性磷酸酶(t=11.134)、血清总胆红素(t=1.653)、血清直接胆红素(t=1.677)及谷氨酰转肽酶(t=13.537)高于治疗后,差异有统计学意义(P<0.05)。治疗后两组相比较,对照组谷丙转氨酶(t=1.655)、碱性磷酸酶(t=1.982)、血清总胆红素(t=1.753)、血清直接胆红素(t=1.688)及谷氨酰转肽酶(t=2.101)高于治疗组,差异有统计学意义(P<0.05)。两组比较,治疗组治疗后血清抗CMV-IgM(χ^2=8.076)及CMV-DNA(χ^2=22.634)转阴率较高,差异有统计学意义(P<0.01)。结论双环醇联合更昔洛韦、熊去氧胆酸治疗婴儿巨细胞病毒性肝炎能够降低谷丙转氨酶、血清总胆红素及碱性磷酸酶,降低胆汁酸水平,促进肝细胞恢复,效果显著,能缩短病程,加速恢复。
【Objective】To evaluate the therapeutic effects of bicyclol combined with ursodeoxycholic acid and ganciclovir on infantile cytomegalovirus hepatitis.【Methods】A total of 120 infants who were hospitalized and diagnosed with cytomegalovirus(CMV)hepatitis from September 2016 to August 2018 were randomly divided into observation group(60 cases)and control group(60 cases).Both groups were given ganciclovir injection at 5 mg/kg per time,2 times per day for 2 weeks,and ursodeoxycholic acid orally at 5 mg/kg per time,2 times per day.In the treatment group,1mg/kg bicyclic alcohol tablets were added,2 times per day.【Results】In the observation group,pre-treatment alanine aminotransferase(ALT)(t=2.000),alkaline phosphatase(t=12.321),serum total bilirubin(t=2.321),serum direct bilirubin(t=5.543)and glutamyl transpeptidase(t=15.432)were higher than those after treatment,and the differences were statistically significant(P<0.05).In the control group,before treatment,ALT(t=1.913),alkaline phosphatase(t=11.134),serum total bilirubin(t=1.653),serum direct bilirubin(t=1.677)and glutamyl transpeptidase(t=13.537)were higher than those after treatment,and the differences were statistically significant(P<0.05).Compared between the two groups after treatment,the control group was significantly higher than the observation group in terms of ALT(t=1.655),alkaline phosphatase(t=1.982),total bilirubin(t=1.753),direct bilirubin(t=1.688)and transpeptidase(t=2.101).After treatment,serum anti-CMV-IgM(χ^2=8.076)and CMV-DNA(χ^2=22.634)were significantly higher in the observation group(P<0.01).【Conclusion】Bicyclol combined with ursodeoxycholic acid and ganciclovir can reduce glutamic pyruvic transaminase,alkaline phosphatase and serum total bilirubin,and decrease bile acid levels to lessen liver cell damage and promote the recovery of liver cells.It can shorten the course of disease and accelerate the recovery.
