摘要
目的股骨颈骨折老年患者应用全髋关节置换与骨折内固定手术治疗效果情况分析。方法选取2018年6月-2019年6月该院诊治90例股骨颈骨折老年患者临床资料,随机分两组,对照组予以骨折内固定手术,研究组予以全髋关节置换方案,分析两组疗效相关指标、髋关节相关功能及并发症情况。结果治疗后,研究组出血量(107.06±16.68)mL、手术时间(52.58±12.92)min、住院时间(18.03±3.46)d及卧床时间(13.35±3.78)d,均比对照组少,差异比较具统计意义(t=29.567、15.634、21.240、8.766 <0.05);研究组髋关节相关功能(90.36±3.46)分比对照组(79.25±4.78)分高(t=12.630,P<0.05);研究组假体松动、肺感染、骨不连等并发症4.44%比对照组20.00%少,比较差异具统计意义(χ^2=5.074,P<0.05)。结论股骨颈骨折老年患者应用全髋关节置换,相比骨折内固定手术治疗,更能有效减少出血量及症状恢复时间,改善患髋关节相关功能,并降低并发症发生,具一定临床应用价值。
Objective To analyze the effect of total hip arthroplasty and fracture internal fixation on elderly patients with femoral neck fractures. Methods The clinical data of 90 elderly patients with femoral neck fractures diagnosed and treated in the hospital from June 2018 to June 2019 were randomly divided into two groups. The control group was treated with fracture internal fixation. The research group was treated with total hip replacement. The two groups were analyzed of the efficacy-related indicators, hip-related functions, and complications. Results After treatment, the blood loss in the study group(107.06±16.68)mL, operation time(52.58±12.92)min, length of hospital stay(18.03±3.46)d, and bed time(13.35±3.78) d were all less than those in the control group, with differences. It was statistically significant(t=29.567,15.634,21.240,8.766,P<0.05);hip-related functions(90.36±3.46)points in the study group were higher than those in the control group(79.25±4.78)points(t=12.630,P<0.05);complications such as loosening of the prosthesis, lung infection, and bone nonunion in the study group were 4.44% less than the 20.00% in the control group, and the difference was statistically significant(χ^2=5.075,P<0.05). Conclusion Total hip joint replacement in elderly patients with femoral neck fractures can effectively reduce the amount of bleeding and symptom recovery time, improve the related functions of the hip joints, and reduce the occurrence of complications.
